无气腹腹腔镜手术治疗卵巢良性囊肿

摘　　要：目的:通过比较3种手术方式对卵巢良性囊肿患者临床情况的影响,探讨无气腹腹腔镜手术在治疗卵巢良性囊肿中的优缺点。方法:60例卵巢良性囊肿患者随机分为传统开腹组、气腹式腹腔镜组和无气腹腹腔镜组,比较3组术中平均气道压(MAP)、心率(HR)、动脉血氧饱和度(SPO2)、呼吸末CO2分压(PetCO2)、肺泡与口之间压差(PAW)、肺顺应性(COMPL)等呼吸循环指标及术中出血、术后肛门排气时间、住院天数、住院费用等指标。结果:无气腹腹腔镜组具有传统开腹组术中呼吸循环指标平稳的优点,同时比气腹式腹腔镜组术中出血少、术后肛门排气早、住院时间短、住院费用少。结论:无气腹腹腔镜下卵巢良性囊肿剥出术是一种较好的治疗妇科疾病的微创技术。Objective：To study the the influence of three operation styles in the patients with benign ovarian cysts. Methods： Sixty patients with final diagnoses of benign ovarian cysts,randomly divided into three groups,including laparotomy group, gas laparoscopic group and gasless laparoscopic group. The indexes including the index of respiration and circulation system and volume of blood loss during the operation, the time of passage of gas by anus after the operation,the hospital day, cost of hospitalization were compared. Results：Gasless laparoscopic group possess the same good quality as the laparotomy group , such as steady of the function of respiration and circulation system during the operation,at the same time ,related to the gas laparoscopic group, possess the good quality such as little blood loss. Passage of gas by anus after the operation was early,the fewer hospital day,the less cost of hospitalization. Conclusion： Gasless laparoscopic surgery of oophorocystectomy is a better treatment technic with minimally invasion to gynecological disease.it desere clinical extending.
