摘  要:目的探讨吸烟与非小细胞肺癌(NSCLC)p21ras蛋白和FHIT蛋白表达异常的关系,揭示吸烟致肺癌作用机制及靶点。方法应用免疫组化SP法检测80例NSCLC癌组织及20例正常肺组织中p21ras蛋白和FHIT蛋白表达情况,分析患者吸烟因素对p21ras蛋白和FHIT蛋白表达的影响。结果80例NSCLC标本中p21ras蛋白高表达和FHIT蛋白低表达,而正常肺组织则相反,两组比较差异均有统计学意义(P<0.01)。NSCLC患者中吸烟组的p21ras蛋白表达明显高于不吸烟组,FHIT蛋白表达则明显低于不吸烟组,分别二组比较,差异有统计学意义(P<0.01)。并且随着NSCLC患者的吸烟指数(BI)的升高p21ras蛋白表达渐增强和FHIT蛋白表达渐减弱,不同BI组之间的两者异常表达均有统计学意义(P<0.01)。肺腺癌和大细胞肺癌中吸烟与p21ras蛋白高表达关系密切,而肺鳞癌中吸烟与FHIT蛋白异常表达关系密切,均有统计学意义(P<0.01)。NSCLC患者p21ras蛋白和FHIT蛋白异常表达无明显的相关性,但二者在NSCLC组织的异常变化存在差异(P<0.01)。结论p21ras蛋白和FHIT蛋白异常表达与NSCLC发生密切相关,在吸烟致肺癌发病中起重要作用,且可能为烟草致癌的关键步骤及分子靶点,吸烟是肺腺癌、大细胞肺癌p21ras蛋白高表达和肺鳞癌中FHIT蛋白低表达的一个重要因素。Objective To study the relationship between the abnormal expression of p21^ras protein and FHIT protein and Cigarette smoking in no-small-cell lung cancer(NSCLC) tissue. Methods Immunohisto-chemistry(SP) was used to detect the abnormal expression of p21^ras protein and FHIT protein in 80 cases NSCLC and 20 cases of normal control lung tissue, studying the influences of Cigarette smoking on the abnormal expression of p21^ras protein and FHIT protein. Results High expression of p21^ras protein and low expression of FHIT protein were found in NSCLC tissue oppsite results were found in normal control lung tissue. Significant difference was found between two groups( P 〈 0.05). Expression of p21^ras protein in Cigarette smoking group was higher than that of no smoking groups; Expression of FHIT protein in Cigarette smoking group was lower than that of no smoking groups. Significant difference was found between two groups( P 〈 0.05). BI of NSCLC patient was related to abnormal expression of p21^ras protein and FHIT protein. Significant difference was found between differrent BI groups( P 〈 0.05 ). High expression of p21^ras protein had close correlatetion with Cigarette smoking in lung adenocarcinoma and large cell lung cancer, and low expression of FHIT protein had close correlatetion with Cigarette smoking in lung squamous cell carcinoma. No relevance was found betwwen high expression of p21^ras protein and low expression of FHIT protein in NSCLC,but Significant difference was found between them. Conclusion The abnormal expression of p21^ras protein and FHIT protein had close correlatetion with tumorigenic development of lung cancer, and played an important role in the process of lung cancer induced by Cigarette smoking. K-ras and FHIT became the key step and molecule target of smoke cancefigenic. Cigarette smoking was an important factor that causeed high expression of p21^ras protein in lung adenocarcinoma and and large cell lung cancer, that causeed FHIT protein abnormal expression in lung
