摘　　要：目的探讨重组腺病毒介导的超级白细胞介素6(Hyper-IL-6)对大鼠急性肝衰竭的治疗作用及机制。方法80只急性肝衰竭模型大鼠随机分为:未感染组、Ad0组、AdIL-6组、AdHIL-6组,分别予腹腔注射生理盐水、Ad0、AdIL-6、AdHIL-6,后进行肝功能指标、肝脏病理学及肝细胞原位凋亡检测,并观察增殖细胞核抗原(PCNA)、caspase-3在肝组织中的表达情况及大鼠14d存活率。结果AdHIL-6组血清丙氨酸氨基转移酶(ALT)、总胆红素(TB)含量及凋亡指数明显降低,而肝组织PCNA表达明显增高(P<0.01),肝组织病变减轻,caspase-3表达阳性,14d存活率较高。结论Hyper-IL-6对急性肝衰竭大鼠有明显的促肝细胞增殖及抗肝细胞凋亡效应,能有效改善肝功能及肝组织学状况,提高肝衰竭大鼠存活率。Objective To explore the clinical effects and mechanisms of Hyper-IL-6 mediated by recombinant adenovirus on rat with acute liver failure （ALF）. Methods Eighty ALF rats were randomly divided into 4 groups, normal saline was injected for non-infection group;Ad0, AdIL-6 and AdHIL-6 was respectively injected in group AdO, group AdIL-6, group AdHIL-6. Subsequently the index of hepatic function, liver pathology, hepatocelluar apoptosis in situ and the expression of PCNA and caspase-3 in liver tissue were examined. The survival rate of rats for 14 days was recorded. Results The level of serum ALT and TB, and apoptotic index in rats of group AdHIL-6 was markedly decreased but expression of PCNA increased （P 〈0. 01 ）. The liver pathological change was mild, and owned higher survival rate at the end of 14 days in group AdHIL-6. Conclusion Hyper-IL-6 enhances hepatocelluar proliferation and protects hepatocyte against apoptosis. It can effectively ameliorate liver function and improve the survival of rats with ALF.
