摘 要:目的探讨肝细胞肝癌(HCC)并门静脉癌栓(PVTT)组织中血小板衍生生长因子受体(PDGFR)的表达与微血管密度(MVD)的相关性及其临床意义。方法采用免疫组织化学方法检测36例HCC并PVTT组织、12例正常肝脏标本中PDGFR的表达与MVD计数。结果HCC并PVTT组织中PDGFR的表达阳性率及MVD计数明显高于癌旁组织及正常肝组织(2χ=4.732、6.321,t=5.432、6.852,P〈0.05)。PDGFR在TNMⅢ~ⅥA期肿瘤组织中的阳性率明显高于TNMⅡ期(P=0.008)。分化差、肿瘤直径≥5 cm和TNMⅢ~ⅥA期肿瘤组织MVD计数明显高于分化好、肿瘤直径〈5 cm和TNMⅡ期者(t=5.842~6.764,P〈0.01)。PDGFR表达阳性肿瘤组织的MVD明显高于PDGFR表达阴性组织(t=7.236,P〈0.01)。结论HCC并PVTT中PDGFR表达上调,可能通过促进肿瘤血管形成而参与HCC并发PVTT的发生和发展。Objective To study the expressions of platelet-derived growth factor receptor (PDGFR) in hepatocellular carcinoma (HCC) and portal vein tumor thromhus (PVTT) and their relationship with the mierovessei density (MVD). Methods Expressions of PDGFR were investigated via immunohistochemical technique in 36 cases of HCC and PVTT and 10 of normal con- trois. The MVD was measured. Results The PDGFR positive staining rate and MVD in HCC and PVTT tissues were significantly higher than those in the adjacent liver tissue and normaitlssues (x^2=4.732,6.321;t=5.432,6.852;P〈0.05). The expression rate of PDGFR was significantly higher in TNM Ⅲ-- ⅣA stage than in TNM Ⅱ (P=0. 008). MVD was significantly higher in poorly differentiated,larger tumor (≥ era) and TNM Ⅲ-- Ⅳ A stage than in well differentiated, smaller tumor (〈5 cm) and TNM Ⅱ stage (t=5.8-12-6. 764,P〈0.01). MVD in the PDGFR positive tissue was also significantly higher than that in the PDGFR negative ones (t= 7. 236 ,P〈0.01 ). Conclusion Over expression of PDGFR in HCC and PVTT tissue may he associat ed with the pathogenesis of HCC and PVTT through increasing neovascularization.
