摘  要:目的探讨p53、ER和PR在98例乳腺癌中的表达及意义。方法采用组织芯片平台,应用免疫组化方法检测ER、PR和p53在98例乳腺癌和26例癌旁正常乳腺组织中的表达状况。结果p53在乳腺癌中阳性表达率为28.6%,与肿瘤的组织学分级相关,p53在乳腺癌中的表达高于癌旁乳腺组织(P=0.008);乳腺癌中ER和PR的表达呈显著正相关,两者分别与组织学分级呈负相关,ER和或PR阳性表达者生存率高,且乳腺癌中PR的表达低于癌旁正常组织(P=0.027)。多因素回归分析中,PR和临床分期与生存相关。结论p53与乳腺癌的发生和分化密切相关,ER和PR与乳腺癌的发生、分化和预后密切相关,三项指标的联合检测可用来指导临床用药和判断预后。Aim To investigate the expression of p53, ER and PR in breast cancers and its clinical significance. Methods p53, ER and PR were detected by immunohistochemistry with tissue microarrays which contained 98 breast cancers and 26 cases of breast tissues adjacent to cancers. Results p53 was highly overexpressed(28.6% ) in breast cancers,which was correlated with histological grade. PR was downregnlated in breast cancers compared to the normal breast tissues adjacent to cancers(P = 0. 027). PR expression was remarkably correlated with ER status in breast cancers. They were respectively inversely correlated with histological grade. Kaplan-Meier survival curves showed ER, PR, stage, tumor size and lymph node status were respectively correlated with survival. The result of multivariate Cox regression analysis for overall survival showed that PR and stage were all significant prognostic predictors. Conclusion ER, PR and p53 may play a role in disease pathogenesis of breast cancer, besides they were associated with differentiation of breast cancer, and ER and PR were closely associated with prognosis.
