超声造影在临床诊断小肾癌中的价值

摘　　要：目的:探讨超声造影诊断小肾癌的临床价值。方法:回顾性分析2005年12月～2007年3月收治的36例肾脏占位性病变(病灶直径<3cm)患者的增强CT、超声造影检查资料,并与病理检查结果进行对照研究。结果:超声造影在鉴别诊断肾脏良恶性病变的敏感性为94.4,特异性为100,准确性为97.2;增强CT鉴别诊断肾脏良恶性病变的敏感性、特异性和准确性分别为66.7、100、83.3。经病理检查证实的18例小肾癌患者的超声造影表现为肿块内造影剂快速充填并早于自身肾皮质,造影剂充填强度也明显高于自身肾皮质,造影剂消退时间迟于自身肾皮质。造影剂早充填、高增强、慢消退是本组小肾癌的超声造影特点。结论:超声造影相比增强CT,在诊断小肾癌的敏感性与准确性方面差异有统计学意义(P<0.05),超声造影具有更高的诊断敏感性和准确性。Objective：To investigate the clinical value of ultrasonic contrast （USC） in the diagnosis renal carcinoma. Methods：Between 2005. 12-2007.3, thirty-six patients with small renal lesions （the diameter of renal lesions less than 3 cm） were collected in our department. UCS and contrast-enhanced computed tomography were carried out for all patients,and then contrast study on the examination of pathology. Results：The sensitivity,specificity and accuracy of USC was 94.4%, 100%, 97.2%, respectively. Those of contrast-enhanced computed tomography was 66.7% ,100% ,83.3%, respectively. The lesions of the 18 small renal carcinomas which were confirmed by pathology showed marked enhancement of flow filling quickly after intravenous contrast agent and it faster than the renal parenchyma, ;The filling intensity of contrast agent in the lesions was significantly stronger than that in the renal parenchyma too. The contrast agent fill early, increase strongly and clear slowly which was the characteristic of USC in these small renal carcinoma. Conclusions： There was significance （P〈0.05） of sensitivity, specificity and accuracy in differentiating small renal carcinoma by USC and contrast-enhanced computed tomography. USC helped to increase the delectability of small renal carcinoma.
