摘 要:目的观察膈下逐瘀汤对实验性肝硬化大鼠血液流变学的影响。方法将50只SD大鼠随机分为5组,正常对照组、模型对照组、膈下逐瘀汤低剂量组、膈下逐瘀汤高剂量组、阳性药对照组各10只。除正常对照组外,其余各组采用四氯化碳(60%CCl4)、普通饮食及乙醇建立大鼠肝硬化模型。造模成功后,低剂量组每日用药剂量为1.3g/kg(相当于成人剂量的10倍),每日1次灌胃;高剂量组每日用药剂量为2.6g/kg(相当于成人剂量的20倍),每日1次灌胃;对照组用复方鳖甲软肝片的混悬液灌胃,用药剂量1.2g/kg(相当于成人剂量的20倍),每日1次灌胃。正常对照组、模型对照组予0.9%氯化钠注射液,灌胃容积均为1mL/只,每日1次,连续灌药4周。检测各组大鼠血液流变学各项指标,并进行统计学分析。结果模型组大鼠全血黏度(高切)、全血黏度(低切)、血浆黏度、红细胞比积、纤维蛋白原较正常对照组明显降低(P<0.01);红细胞沉降率、血小板聚集率、红细胞聚集指数明显升高(P<0.01)。用药后,膈下逐瘀汤高、低剂量组和阳性药对照组大鼠全血黏度(高切)、全血黏度(低切)、血浆黏度、红细胞比积、纤维蛋白原与模型对照组比较均明显升高(P<0.05,P<0.01),红细胞沉降率、血小板聚集率均明显降低(P<0.01),其中,膈下逐瘀汤高、低剂量组与阳性药对照组比较差异有统计学意义(P<0.05,P<0.01)。结论膈下逐瘀汤可有效地纠正"低凝高聚"这一血液流变学的异常改变,进而改善肝脏微循环,从而达到治疗肝硬化的目的。Objective To observe the influence of Gexiazhuyu decoction on hemorrheology in rat with experimental cirrhosis. Methods 50 rats were divided into five groups (including normal control group, model control group, Gexiazhuyu decoction low dose group, Gexiazhuyu decoction high dose group and Compound Biejiaruangan tablets group). 60% CCl4, general diet and ethanol were adopted to four groups except normal control group for establishing model of cirrhosis. Corresponding drugs were lavaged in each groups respectively. 0.9% NaCl injection was adopted to normal control group and model control group. Volume of intragastric administration was 1 ml/rat, once a day. Indexes of hemorrheology (whole blood viscosity in high shear rate, whole blood viscosity in low shear rate, plasma viscosity, hematocrit, and fibrinogen ) were analyzed in each groups. The experimental course was four weeks in five groups. Results Whole blood viscosity in high shear rate, whole blood viscosity in low shear rate, plasma viscosity, hematocrit and fibrinogen in model control group obviously decreased compared with those in normal control group ( P 〈 0.01). Erythrocyte aggregation, platelet aggregation and blood sedimentation rate in model control group obviously increased compared with those in normal control group ( P 〈 0.01). After administration whole blood viscosity in high shear rate, whole blood viscosity in low shear rate, plasma viscosity, hematocrit and fibrinogen in high dose group, low dose group and normal control group obviously increased com- pared with those in normal control group ( P 〈 0.05, P 〈 0.01 ). Platelet aggregation and blood sedimentation rate in high dose group, low dose group obviously decreased compared with those in normal control group ( P 〈 0.01). Conclusion Gexiazhuyu decoction can effectively correct dysfunction changes of Hemorrheology on viscosity and aggregation, and improve microcirculation of liver and treat cirrhosis.
