摘 要:目的评估饮食脂肪和热量对健康人胃食管酸反流的影响。方法选择25例健康志愿者,先做食管测压,先后2次服用低脂肪餐和高脂肪餐,每次记录餐后6h食管pH值。结果健康志愿者在2种热量相等的低脂肪和高脂肪含量的试餐后,pH<4总反流时间百分率(2.83±3.33%,3.15±3.05%),反流时间>5min的反流次数(0.32±0.627,0.4±0.866)及反流次数(15.96±15.54,16.6±14.07)的比较,均没有显著性差异(P>0.05)。结论在同热量含量下,食物中脂肪含量对健康人餐后6h酸反流没有影响。Objective To assess the effects of equiealorie low and high fat meals on esophageal acid exposure over a 6- hour postprandial period in healthy volunteers. Methods Twenty-five healthy volunteers( 18 men,7 women,20 to 60 years) , underwent 6-hour oesophageal pH monitoring after 2 solid/liquid meals with the same volume and osmolarity on separate days in a randomized order:high fat(60% fat) 2.8 MJ;balaneed(25% fat) 2.8 MJ. Results No difference was observed between the low and the high fat meal for the mean percentage of time at pH 〈4(2.83±3.33% vs 3. 15±3.05% , P 〉0. 05) ,the number of reflux episodes ( 15.96 ± 15.54 vs 16. 6 ± 14.07, P 〉 0.05 ) and number of over 5 minute reflux episodes (0. 32 ± 0. 627 vs 0. 4 ± 0. 866, P 〉 0. 05 ) in healthy volunteers. Conclusion In healthy volunteers no difference in postprandial acid exposure was seen after a high fat meal eomparet with an isoealorie low fat meal for at least six hours. Our results suggest that advice on dietary habits in patients with GERD should be concentrated on decreasing the calorie load of meals rather than their fat content.
