摘 要:目的探讨使用强制性运动疗法(CIMT)对脑卒中偏瘫患者下肢运动功能康复的疗效。方法选取脑卒中偏瘫患者30例,随机分为治疗组和对照组,每组15例。对照组采用常规的物理疗法为主的Bobath技术治疗。治疗组采用CIMT治疗,两组患者均治疗8周。比较两组患者治疗前后10m最大步行速度、Berg平衡量表、简式Fugl-Meyer评价量表中的下肢评价及Barthel指数的变化。结果两组患者治疗前各项指标差异均无统计学意义(P>0.05),两组患者治疗后与治疗前比较,各项功能指标变化差异有统计学意义(P<0.05)。治疗组各项指标明显优于对照组(P<0.05)。结论CIMT改善脑卒中偏瘫患者步行速度和提高日常生活能力具有重要意义。Objective To explore the efficacy of constraint-induced movement therapy(CIMT)for motor function recovery of lower-extremity of stroke patients. Methods Thirty patients with stroke were divided into two groups.treatment group and control group. The control group adopted routine treatment with Bobath technique. the treatment group was treated with CIMT. The two groups were treated for four hours every day, five times a week for eight weeks. The therapeutic efficacy was separately evaluated with maximum walking speed, Berg balance scale and FuglMeyer scale for evaluating lower-extremity function before treatment and eight weeks after treatment. Ability of function of daily life was evaluated with Barthel index. Results Before treatment, the difference has no statistical significance between the 2 groups. After CIMT treatment, the difference was very obvious compared with before treatment. Comparison of the two groups showed that the treatment group was obviously better than the control group (P 〈0.05). Conclusion CIMT may be an efficacious method for improvement of walking speed and ability of daily life in stroke patients.
