基于中风痉挛性瘫痪患者报告的临床结局评价量表的信度、效度分析

摘　　要：目的评价"基于中风痉挛性瘫痪患者报告的临床结局评价量表"的信度和效度。方法采用现场调查的方式,由中风痉挛性瘫痪患者填写该量表,收回有效量表106份。进行了分半信度、Cronbach's Alpha系数分析,结构效度和区分效度分析。结果量表分半信度为0.9396,Cronbach's Alpha系数为0.9003;躯体症状、心理状态、社会交往3个维度的信度系数均高于0.75。结构效度采用因子分析法,得出"主观症状"、"客观症状"、"心理感受"、"社会交往"4个领域。中风痉挛性瘫痪患者和中风迟缓性瘫痪患者除"社会交往"领域外,在其他领域和量表总得分差异均有显著性,显示有一定的区分效果。结论该自评量表具有较好的信度和效度,可作为中风痉挛性瘫痪患者临床效果评定方面的工具推广应用。Objective To make the detection on the reliability and validity of the Evaluation Scale of Patient-Reported-Outcome by windstroke patients with spastic paralysis. Methods The way of spot investigation was adopted. The windstroke patients with spastic paralysis were asked to fill in this questionnaire for two times with an interval of 2-3 weeks. Of the questionnaire collected back, those of 106 patients were valid ones. For reliability, the analyses were made on the split-half reliability and Cronbach＇s Alpha coefficient. For validity, the analyses were made on the construct validity and differential validity. Results The total split-half reliability of the data of two times was 0. 9047 and 0. 9396 respectively, and the Cronbach＇s Alpha coefficient 0. 8741 and 0. 9003 respectively. The dimensional reliability coefficients of somatization, mentation, and social communication were all higher than 0. 75. As to construct valid- ity, the factor analysis made the ＂subjective symptom＂ , ＂objective symptom＂,＂psychological feeling＂, and ＂social communication＂ obtained. Except for the ＂social communication＂, there were significant difference in other domains and total score of evaluation between the windstroke patients of spastic paralysis and flaccid paralysis, showing a certain differential effect. Conclusion This self evaluation scale owns a good reliability and validity, so it can be served as the tool for evaluating the clinical effect of windstroke patients with spastic paralysis.
