髁上型内锁髓内钉在恶性骨肿瘤切除后骨重建术中的应用

摘　　要：目的介绍一种新设计的髁上型内锁髓内钉，适用于股骨下端和胫骨上端同种异体关节移植或瘤段灭活再植术。方法应用髁上型内锁髓内钉于同种异体骨关节移植６例。６例均为恶性骨肿瘤，其中股骨下端４例，胫骨上端２例。该钉为中空，闭合断面呈三叶草形，外径为１０ｍｍ～１２ｍｍ，远端前屈角为８°，近、远端设冠状方向直径５ｍｍ锁孔，逆行插钉法安装。结果６例骨关节移植均未出现近、晚期并发症，４例术后随访６个月以上，均在３个月左右骨接合端有初步愈合，术后功能好。结论髁上型内锁髓内钉使用方便，固定可靠，有较好的临床应用价值。PURPOSE A study of the initial experience in limb salvage surgery with the use of a retrograde supercondylar intramedullary (IM) nail for the treatment of malignant bone tumors is presented.METHODS Since 1995, the authors have performed 6 resections of malignant tumors of extremities, 4 at distal end of femur and 2 proximal end of tibia. The resections were followed by reconstructions with implantations of segmental allografts which used the newly developed retrograde IM rod. The supracondylar IM nail is a fully cannulated closed clover leaf crosssection single piece stainless steel nail with an outer diameter of 11 mm～20 mm, it has an 8° apexanterior bend near the distal end to accomdate the geometry of posterior aspect of the femoral and tibia condyles.RESULTS The 6 cases were followed up for over 6 months, none had early or late complications, and the results were excellent.CONCLUSIONS This device is easy to use in limb salvage surgery, especially in malignant tumors of distal end of femur and proximal end of tibia.
