脑出血微创清除术应用低温复合冲洗液对血肿周围水肿带的影响

摘　　要：目的观察高血压脑出血患者微创术中应用低温复合冲洗液对血肿周围水肿面积及神经功能的影响。方法高血压脑出血患者50例,随机分为治疗组和对照组各25例,治疗组在微创术中应用低温复合冲洗液(冰生理盐水、地塞米松、纳洛酮)冲洗,对照组应用常温生理盐水冲洗,2组均行脑出血的常规治疗,观察2组患者第7、15d与第1d病灶周围水肿面积及神经功能缺损评分之差。结果2组病人术后第1d与第7、15d头颅CT上最大灶周水肿面积之差(治疗组1.02±0.06与1.86±0.12,对照组0.53±0.04与0.61±0.07)及神经功能缺损评分之差(治疗组5.51±1.12与8.24±2.14,对照组2.16±1.32与3.56±1.26)均有显著性差异(P<0.01)。结论低温复合冲洗液能明显减小微创术后病灶周围水肿面积,使神经功能恢复水平进一步提高。Objective To observe the effect of low temperature complex wash liquid on perilesional edema of hematoma and neurological function in patients with hypertensive cerebral hemorrhage in micro scar operation. Methods Fifty patients were randomly divided into two groups, 25 cases in therapeutic group and 25 cases in control group. Therapeutic group were washed with low temperature complex wash lipuid （ice nomal saline , dexamethasone , naloxone） in the treatment of micro scar operation, control group were washed with normal temperature normal saline. Both groups were taken routine treatment for cerebral hemorrhage. The differences of perilesional edema and Chinese stroke scale scores between the two groups at the lst,7th and 15th day were observed. Results There were significant differences on the largest difference of cer.ebral edema area around the hemorrhage focus on CT （therapeutic group 1.02±0.06 and 1.86±0.12,control group 0.53±0.04 and 0.61±0.07）and the difference of grade point for dysfunction nerves （therapeutic group 5. 51±1.12 and 8. 24 ± 2. 14, control group 2.16 ± 1.32 and 3.56 ± 1.26）on the lst and 7th, 15th day after the operation（P〈0.01）. Conclusion Low temperature complex wash liquid can reduce perilesional edema area obviously after micro scar operation and improve the neurological function in patients with hypertensive cerebral hemorrhage.
