人胃液表皮生长因子在消化性溃疡发生与愈合中的作用

摘　　要：以１１例胃溃疡（ＧＵ）和１９例十二指肠溃疡（ＤＵ）患者为研究对象，观察患者以洛赛克治疗前后胃液中表皮生长因子（ＥＧＦ）含量的变化。结果发现，治疗前ＧＵ和ＤＵ患者胃液ＥＧＦ含量明显低于正常人，分别为１４８．２±１５９．９ｐｇ／ｍｌ和１４７．８±２０４．１ｐｇ／ｍｌ。经治疗后，所有ＧＵ患者胃液ＥＧＦ含量恢复正常，同时溃疡愈合；部分ＤＵ患者在溃疡愈合后其胃液ＥＧＦ的含量增加，而溃疡未愈者胃液ＥＧＦ含量也不增加。表明胃液ＥＧＦ与消化性溃疡的发生和愈合有关，并提示胃液ＥＧＦ含量持续低下可能与ＤＵ易复发有关。To observe the changes of human gastric juice EGF before and after treatment with Losec, 11 patients with active benign gastric ulcers and 19 with active duodenal ulcers were studied. The results showed that the concentrations of gastric juice EGF(before treatment) were consistently reduced, compared with those found in normal control subjects(511.9±54.1pg/ml), both in gastric ulcer(148.2±159.9pg/ml) and duodenal ulcer patients(147.8±204.1pg/ml)(P<001 and P<005, respectively). EGF values distinctly lower than in the controls were still present in the gastric juice of patients with healed duodenal ulcers(310.0±233.2pg/ml,P<005) and no change in those patients with unhealed duodenal ulcers(127.3±65.4pg/ml) after Losec therapy. Whereas no difference was observed in patients with healed gastric ulcers(514.0±309.8pg/ml) after treatment with Losec. All the results demonstrated that gastric juice EGF deficiency may have a role on the pathogensis and healing of peptic ulcer disease. The consistently reduced gastric juice EGF in patients with duodenal ulcer before and after Losec therapy maybe related to the recurrence of duodenal ulcer disease.
