膀胱全切去结肠带盲升结肠新膀胱术疗效观察

摘　　要：目的探讨膀胱全切去结肠带盲升结肠新膀胱术的临床疗效。方法对32例男性膀胱癌患者行膀胱全切去结肠带盲升结肠新膀胱术，膀胱全切采用顺逆相结合的方法，截取盲肠、升结肠、切除结肠带作为新膀胱。结果手术时间250～360min，平均270min。术后全部获得随访，31例白天夜间均可控制排尿，只有1例夜间不能控制排尿；2例单侧轻度肾积水；2例新膀胱结石，其中1例反复尿路感染；1例尿道残端癌复发；1例新膀胱后尿道吻合口狭窄；1例肠梗阻。结论膀胱全切去结肠带盲升结肠新膀胱术时间短，并发症少，无电解质紊乱，更符合生理要求。Objective It is to discuss the clinical effect radical cystectomy and detenia ceacal-eolon orthotopic neobladder. Methods 32 male patients with bladder carcinoma were performed on with antegradly and retrogradly radical cystectomy and detenia ceacal-colon orthotopic neobladder. Radical cystectomy was carried out by combined antegrade and retrograde technique, and the neobladder was made by a section of detenia ceacal-colon reserved its mesentery. Results The operative time took 250 - 360 min with a mean of 270 min, All the patients had been followed up. 31 cases had achieved excellent day and night time continence, Only one had nighttime incontinence, Two cases suffered from slight single hydronephrosis. Neobladder stone occurred in two cases, one of which had been repeatedly found urinary tract infection, Anastomotic stricture between neobladder and urethra, cancer recurrence of urethral stump and intestinal obstruction occurred in one case respectively. Conclusion Radical cystectomy and detenia ceacal-eolon orthotopic neobladder is according with physiological requirement with shorter operation time, fewer complication, and with no electrolyte disturbance.
