三级甲等综合医院护理工作量测量项目的框架构建

摘　　要：目的确定三级甲等综合医院护理工作量测量的项目,为护理工作量测量奠定基础,从而优化护理人力配置。方法在文献综述、各科室护士长提供护理工作项目的基础上,自行设计第1轮护理工作项目调查表,由护士长组织科室护士进行项目符合程度评分,重新筛选项目后,再组织20名护理专家集中讨论,得到最终框架。结果第1轮调查护理工作项目为341项(基础护理中直接护理项目84项、间接护理项目72项;专科护理中直接护理项目163项、间接护理项目22项);最终确定项目为226项(基础护理中直接护理项目47项、间接护理项目28项;专科护理中直接护理项目125项、间接护理项目26项),其中基础护理中间接护理10项为非护理专业性工作,专科护理中间接护理7项为非护理专业性工作。结论三级甲等综合医院护理工作量测量项目范围扩大,非护理专业性工作较多,需改革护理人力资源配置方法及界定护理工作范畴。Objective To establish the items of nursing workload measurement in top-notch general hospitals, so as to provide scientific information for optimizing the allocation of nursing human resources. Methods On the basis of literature review and items proposed by head nurses in each department, a questionnaire of workload measurement items was formulated and distributed to all departments for scoring. Then we filtered the items again and invited 20 nursing specialists to discuss the left-over items. Results The self-designed questionnaire contained fundamental nursing and specialized nursing, fundamental nursing was composed of 84 direct nursing items and 72 indirect nursing items, and 163 direct nursing items and 22 indirect nursing items belonged to specialized nursing; the final questionnaire was composed of 47 direct nursing items and 28 indirect nursing items in fundamental nursing, and 125 direct nursing items and 26 indirect nursing items in specialized nursing. Ten items of indirect nursing items in fundamental nursing did not belong to nursing profession, nor did 7 in specialized nursing. Conclusion The workload of nurses in top-notch general hospitals has a wide range, so does the non-professional workload. Methods for allocation of nurse staffing should be innovated and the category of nursing profession should be established.
