摘  要:目的通过检测胸腔积液中癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)和细胞角蛋白19(CYFRA21-1)的水平并进行分析,以期为临床胸腔积液诊治提供科学的依据。方法收集恶性胸腔积液30例、结核性胸腔积液18例、类肺炎性胸腔积液23例。应用ELECSYS-2010型全自动电化学发光免疫分析系统定量检测CEA、NSE及CYFRA21-1含量。结果恶性胸腔积液组三种肿瘤标志物含量明显高于结核组和类肺炎组(P<0.01),单项检测中,CYFRA21-1的阳性率(80%)高于其它两个指标(P<0.01),三个指标联合检测阳性率(90%)均显著高于单项检测。结论CEA、NSE及CYFRA21-1肿瘤标志物联合检测可以提高恶性胸腔积液诊断的阳性率,对良恶性胸腔积液的鉴别诊断具有一定的指导意义。Objective To analyze the level of CEA, NSE and CYFRA21 - 1 in pleural effusion so as to provide scientific basis for clinical diagnosis and treatment. Methods The pleural effusion samples were collected from 30 patients with malignant pleural effusion, 18 patients with tuberculous pleural effusion and 23 patients with parapneumonic effusion. The levels of CEA,NSE and CYFRA21 - 1 in pleural effusion were detected by eleetrochemiluminescence immunoassay. Results The levels of three tumor markers in malignant pleural effusion group was significantly higher than tuberculous pleural effusion and parapneumonic effusion (P 〈 0.01 ) ; the positive rate of CYFRA -21 was 80%, higher than CEA and NSE (P 〈 0.01 ), in single detection ;The positive rate of the combination of the three markers was 90.0%, significantly higher than sigule detection. Conclusion The levels of CEA, NSE and CYERA21 - 1 in pleural effusion are valuable for differentiation of benign ones from malignant ones. The combined detection can increase significantly the sensitivity of the test.
