摘  要:目的建立24h快速病原菌鉴定及快速药敏常规检测系统,为临床及时诊疗与控制医院感染提供病原菌学依据。方法采用旋转恒温箱替代普通恒温箱;在增菌培养瓶和药敏平板中加入氯化-2,3,5-三苯基四氮唑(TTC)、琥珀酸盐;加大病原菌生化鉴定管反应底物浓度的同时,增加病原菌接种量。结果阳性培养显示时间比在普通恒温箱及常规培养液中显著缩短(x^2=74.92,P〈0.01);加大菌液浓度,4~5h可获得明显菌落,12h内获得病原菌鉴定结果,2~5h得出可靠药敏结果。结论通过方法学的改进,经过4年的实验室证实,大部分标本均能获得可靠的培养结果,说明在24h之内,快速病原菌鉴定及药敏检测系统可以为临床诊疗和控制医院感染及时提供科学的实验室依据。OBJECTIVE To develop a rapid system for bacteria identification and susceptibility test in 24 hours,and provide bacteriological evidence for the control of nosocomial infection and the timely diagnosis and treatment. METHODS The simple constant temperature box was replaced by revolving constant temperature box~ 2,3,5- triphenyltetrazolium chloride (TTC) and succinate were added to the culture bottle and the culture medium of drug susceptibility;the concentration of the reactive substrate in the bacterial biochemical tube and the number of the inoculated bacteria were increased at the same time. RESULTS The time of positive blood culture in the revolving constant temperature box was significantly shorter than that in the simple one (x2= 74. 92, P〈0. 005). The obvious colonies were seen after 4-5 hours. The bacteria identification results were obtained in 12 hours and the reliable drug susceptibility results were received after 2-5 hours. CONCLUSIONS The reliable culture results are obtained in the great mass of the samples since the method is improved. It is approved in the last four years and it shows that the rapid system for bacteria identification and susceptibility test can provide scientific evidence for the control of nosocomial infection and the timely diagnosis and treatment in 24 hours.
