隧道式小切口小梁切除术对角膜屈光力的影响

摘　　要：目的:对自行设计的隧道式小切口小梁切除术前、后角膜曲率、角膜散光进行研究。方法:选择16例20眼,年龄大于40岁的原发性青光眼患者,进行隧道式小切口小梁切除术,并随访观察,比较术前、术后的垂直向、水平向角膜曲率及角膜散光值、散光性质。结果:术后矫正视力增加或不变18眼,下降2眼。视力下降均为晶状体混浊进展造成;垂直向角膜曲率术后7d;1,2,3mo与术前相比差异无显著性(t=0.44、0.66、0.74、0.59,P值均>0.05);水平向角膜曲率术后7d;1,2,3mo与术前相比差异无显著性(t=0.57、0.64、0.88、0.92,P值均>0.05);角膜散光值术后7d;1,2,3mo与术前相比差异无显著性(t=0.17、0.89、0.96、0.36,P值均>0.05);循规散光和逆规散光的比值在术前、术后7d;1,2,3mo为10∶10、10∶10、8∶10、8∶12、7∶12。结论:隧道式小切口小梁切除术对角膜屈光力没有影响,没有造成术后散光。AIM. To investigate the astigmatism before and trabeculectomy. corneal curvature and corneal after tunnel small-incision METHODS： Sixteen primary glaucoma patients （20 eyes）, who aged over 40 years old, were selected and accepted the tunnel small-incision trabeculectomy. The vertical and horizontal corneal curvature, the value of keratoastigmatism, and the astigmatism peculiarity were investigated for pre- and post-operatively. RESULTS： The corrected vision improved or kept constant in 18 eyes, dropped in 2 eyes. The lens opacity was the cause for hypopsia. The vertical and horizontal corneal curvature had no significant difference 7 days, 1, 2, and 3 months after operation, compared with preoperative. （t=0.44, 0.66, 0.74, 0.59; and t=0.1？, 0.89, 0.96, 0.36, respectively, P 〉 0.05）. The value of keratoastigmatism had no significant difference before and after operation （t=0.17, 0.89, 0.96, 0.36, P〉0.05）. The relative values of regular astigmatism and against-the-rule astigmatism preoperative and 7 days, 1, 2, and 3 months after operation were 10： 10, 10： 10, 8： 10, 8： 12, and 7： 12, respectively. CONCLUSION： The tunnel small-incision trabeculectomy has no influence on cornea refractive power, and does not result in postoperative astigmatism.
