摘 要:目的探讨微染色体维持蛋白2(MCM2)和视网膜母细胞瘤(Rb)基因蛋白在喉鳞状细胞癌(LSCC)中的表达变化及意义。方法采用免疫组化法检测60例LSCC组织、10例喉癌前病变、10例声带息肉及10例LSCC癌周正常喉组织中MCM2与Rb蛋白的表达。结果癌周正常喉组织、声带息肉、喉癌前病变和LSCC组织中MCM2的阳性表达率分别为10.00%、10.00%、30.00%和85.00%,Rb蛋白的阳性表达率分别为80.00%、80.00%、60.00%和36.67%;LSCC中MCM2表达水平明显高于喉癌前病变及癌周正常喉组织(P均<0.05),且与喉癌病理分级、临床分期及淋巴结转移有关(P均<0.05);LSCC中Rb蛋白表达水平明显低于癌前病变及癌周正常喉组织(P均<0.05);LSCC组织中MCM2与Rb蛋白的表达呈负相关(r=-0.542,P<0.05)。结论MCM2的表达升高和Rb蛋白的表达下降与LSCC的发生、发展有关,检测MCM2有助于判定LSCC的恶性程度及预后。Objective To study the expression and clinical significance of minichromosome maintenance protein 2 (MCM2) and retinoblastoma susceptibility gene(Rb) protein in laryngeal squamous cell carcinomas (LSCC). Methods The MCM2 and Rb protein were detected in 60 cases of LSCC, 10 cases of precarcinoma, 10 cases of vocal cord polyps and 10 cases of normal laryngeal tissues by immunohistochemical staining. Results The positive expression rate of MCM2 in normal laryngeal tissues, vocal cord polyps, precarcinoma, laryngeal carcinoma were 113.00%, 10. 00%, 30. 00% and 85.00% respectively. The positive expression rate of MCM2 in LSCC was significantly higller than that in precarcinoma and normal laryngeal tissues(P 〈 0. 05 ), and was positively correlated with pathological grades, clinical stages and lymph node metastases in LSCC(P 〈 0. 05). The positive expression rate of Rb protein in normal laryngeal tissues, vocal cord polyps, precarcinoma, laryngeal carcinoma were 80. 00%, 80. 00%, 60. 00% and 36. 67% respectively. The positive expression rate of Rb protein in LSCC was significantly lower than that in precarcinoma and normal laryn:geal tissues (P 〈 0. 05 ). The expression of MCM2 in LSCC was negatively corelated with Rb protein( r = - 0. 542,P 〈 0. 05 ). Conclusions Over expression of MCM2 and loss of Rb protein were related to the carcinogenesis and development of LSCC. The determination of MCM2 can be an index for estimating the degree of malignancy and prognosis of LSCC.
