现代白内障囊外摘除术后囊膜破裂原因分析及对视力的影响

摘　　要：目的总结分析现代白内障囊外摘除术中及术后囊膜破裂的原因及对视力的影响,以便采取预防措施.方法选择180例(200眼)白内障病人,常规实行白内障现代囊外摘除联合人工晶体植入术.结果200眼中发生后囊膜破裂13眼(6.5%),其中发生于截囊过程中2眼(1.0%),娩核过程中3眼(1.5%),抽吸皮质及后囊抛光时5眼(2.5%),植入人工晶体时3眼(1.5%).结论(1)白内障囊外摘除术+人工晶体植入术时后囊破裂的发生直接影响术后患者的视力恢复.(2)手术时后囊破裂的发生与白内障类型无明显关系.(3)后囊破裂的发生与手术方式、手术技巧等多种因素有关.Objective To Summarize the causes and the affection to visual acuity of post-capsule membrane rupture during and after extracapsular cataract extraction in order to carry out prophylactid measure. Methods 180 cases with cataract （200 eyes） were selected and performed contemporary extracapsular cataract extraction and intraocular lens implantation routinely. Results There were 13 eyes （6.5%） with post-capsule membrane rupture among 200 membrane cutting （15.4%） , membrane polish, and 3 eyes membrane after contemporary eyes. There were 2 eyes with post-capsule membrane rupture occruing in capsule 3 eyes in nucleus childbirth （23.1%） , 5 eyes in cortex suction and post-capsule in intraocular lens implantation extracapsular cataract extraction Conclusions （1） The rupture of post-capsule and intraocular lens implantation directly affect the recovery of patients after the surgery. （2） The rupture of post-capsule membrane after contemporary extracapsular cataract extraction and intraocular lens implantation is not related with the styles of cataract; the propability of the rupture of post-capsule membrane amony the operation processes has significant difference. （3） The rupture of post-capsule membrane is related with many factors such as operational manners and operational skills; the improvement of these will contribute to the decreasement of post-capsule membrane rupture.
