多焦点LASIK后角膜非球面特点分析

摘　　要：目的分析多焦点准分子激光原位角膜磨镶术（Multifoeal LASIK）后角膜非球面特点及其与手术效应的关系。方法对主视眼（对照眼）接受常规LASIK手术，非主视眼（实验眼）接受多焦点LASIK手术。主要检测项目：角膜地形图（分析角膜非球面特点）、对比敏感度、调节幅度、分析术后角膜非球面特点。结果多焦点眼术前调节幅度平均为为（-4．11±1．11）D（-2．00—6．25D），术后6月平均为（-4．95±0．83）D（-3．25～6．50D），术后和术前比，平均增加（0．84±0．61）D（0—2．25D）。手术前后存在统计学差异（P=0．000）。6mm拟合球面Q值，术后6月，实验眼Q值平均为0．081±0．274（-0．53～＋0．53），对照眼为0．464±0．266（-0．02～＋0．99），两组差异有统计学意义（P=0．00）。5mm拟合球面Q值，术后6月，实验眼Q值平均为-0．235±0．386（-0．944-＋0．424），对照眼为0．259±0．328（-0．013～＋0．881），两组差异有统计学意义（P=0．00）。多焦点眼术后较术前向负Q值反转，而对照眼趋向正Q值。结论多焦点LASIK手术后角膜中央区非球面特点是拟调节力产生的根源。Objective To investigate the corneal asphericity after Multifocal LASIK and the effect of asphericity on surgical results. Methods Standard LASIK using Planoscan was performed on the patient/s dominant eye, while multifocal LASIK was performed on the nondominant eye. Routine examinations included corneal topography,contrast sensitivity function,amplitude of accommodation and so on. The effect of asphericity on pseduaccommodation was evaluated. Results At 6 months,the multifocal eyes attained pseudoaccommodatin of 0.84± 0.61 diopters （range,0-2.25D）. Significant difference were noted both Q value of 6mm diameter and Q value of 5mm diameter between two groups. The Q value of 6mm diameter in the multifocal LASIK was 0.081± 0.274 （range,-0.53 to ±0.53） and 0.464± 0.266 （range,-0.02-＋0.99） in the planoscan group. The Q value of 5mm diameter in the multifocal LAS1K was -0.235± 0.386 （-0.944-＋0.424） and 0.259± 0.328 （-0.013-＋0.881） in the planoscan group.The Q value of multifocal group reversed to be more negative than that ofpreoperation, however, the Q value of planoscan group reversed to be more positive. Conclusions Multifocal LASIK maintained the central corneal asphericity which result in pseudoaccommdation.
