摘  要:目的:通过检测良、恶性胸腔积液脱落细胞端粒酶逆转录酶(hTERT)基因的表达情况,探讨其在良、恶性胸腔积液中的诊断价值。方法:采用逆转录-多聚酶链反应法检测39例恶性胸腔积液、16例良性胸腔积液中hTERT基因的表达情况。结果:恶性胸腔积液中hTERT基因的表达率明显高于良性胸腔积液(P<0.05)。检测hTERT基因表达对恶性胸腔积液的敏感性、特异性和诊断符合率分别为87.18%,81.25%和85.45%。结论:hTERT基因参与了恶性胸腔积液的发生、发展过程。采用RT-PCR法检测胸腔积液中hTERT基因表达有助于临床上良、恶性胸腔积液的鉴别诊断。Objective : To investigate the diagnostic significance of the detection of the expression of human telomerase reverse transcriptase (hTERT) gene in benign and malignant pleural effusion. Methods: Telomerase hTERT gene expression of exfoliated cell in pleural effusion of 16 cases with benign pleural effusion and 39 cases with malignant pleural effusion were measured by RT-PCR. Results: Telomerase hTERT gene expression of exfoliated cell in malignant pleural effusion was higher than that in benign pleural effusion ( P 〈 0.05 ). The sensitivity, specificity and diagnostic accordance rate ( % ) of hTERT mRNA was 87. 18%, 81.25% and 85.45%, respectively. Conclusion: Telomerase hTERT gene might involve in pathological process of malignant pleural effusion. The detection of telomerase hTERT gene by RT-PCR is useful to diagnose and distinguish the benign and malignant of pleural effusion.
