摘  要:目的探讨雌激素受体(ER)α、β亚型和Ki-67在不同甲状腺病变组织中的表达及其意义。方法采用免疫组化SP法对2003-2007年收检的93例甲状腺组织蜡块(包括56例甲状腺癌、13例甲状腺腺瘤、14例结节性甲状腺肿和10例正常甲状腺组织)中ERα、ERβ和Ki-67的表达进行检测。结果ERα和ERβ均表达于甲状腺腺上皮细胞的细胞核和细胞质,ERβ在间质中也有表达。ERα在甲状腺癌、甲状腺腺瘤、结节性甲状腺肿和正常甲状腺中的阳性表达率分别为87.5%、69.2%、42.9%和30.0%,其中在甲状腺癌中的表达显著高于结节性甲状腺肿和正常甲状腺(P<0.05)。ERβ在甲状腺癌、甲状腺腺瘤、结节性甲状腺肿和正常甲状腺组织中的阳性表达率分别为82.1%、53.8%、28.6%和20.0%,其中在甲状腺癌中的表达显著高于其他3组(P<0.05)。Ki-67在甲状腺癌、甲状腺腺瘤、结节性甲状腺肿中的表达率分别为69.6%、23.1%、7.1%,而在正常甲状腺组织中呈阴性表达,Ki-67在甲状腺癌中的表达显著高于其他3组(P<0.05)。甲状腺癌ERβ和Ki-67的异常表达与肿瘤分期以及淋巴结转移有关(P<0.05)。ERα、ERβ的表达与Ki-67表达之间无明显相关(P>0.05)。结论ERα、ERβ的表达可能对甲状腺癌发生过程中的细胞增殖具有促进作用。ERβ和Ki-67的异常表达与甲状腺癌的侵袭和转移有关。Objective To investigate the expression and clinical significance of the estrogen receptor (ER) subsets ERα, ERβ and Ki-67 in tissues of different thyroid diseases. Methods The pathological data of 93 patients with thyroid diseases were collected from Jun. 2003 to Jun. 2007, including thyroid carcinoma (56 cases), thyroid follicular adenoma (13 cases), nodular goiter (14 cases) and normal thyroid tissue (10 cases). SP immunohistochemistry was performed to determine the expressions of ERα, ERβ and Ki 67 in the thyroid tissues. Results ERα and ERβ were expressed in the plasma and nuclei of thyroid epithelial cells, and ERβ was also expressed in the stro ma. The positive rates of ERα expression in cases with thyroid carcinoma, thyroid follicular adenoma, nodular goiter and normal thyroid tissue was 87. 5%, 69. 2%, 42. 9% and 30. 0%, respectively, with significant difference between carcinoma and goiter or normal thyroid tissue (P〈0. 05). The positive rate of ERβ expression in cases with thyroid carcinoma, adenoma, nodular goiter and normal thyroid tissue was 82. 1%, 53. 9 %, 28. 6% and 20. 0%, respectively, with significant difference between carcinoma and the adenoma, nodular goiter or normal thyroid tissue (P〈0. 05). The positive rate of Ki-67 expression in cases with carcinoma, adenoma, nodular goiter and normal thyroid tissue was 69. 6%, 23. 1%, 7. 1% and negative, respectively, with significant difference between carcinoma and adenoma, nodular goiter or normal thyroid tissue ( P〈0. 05). The expressions of EI~ and Ki-67 were correlated with TNM stage and lymph node metastasis of thyroid carcinoma (P〈0. 05). No correlation was found between the expressions of ERα, ERβ and Ki-67 (P〉0. 05). Conclusion ERβ and ERα may play an aggravating effect on cell proliferation in the disease course of the thyroid carcinoma. The expressions of ERβ and Ki-67 are related to invasion and metastasis of thyroid carcinoma.
