摘  要:目的探讨汉防己甲素(tetrandrine,Tet)对兔准分子激光屈光性角膜切削术(photorefractive keratectomy,PRK)术后纤维连接蛋白(fibronectin,FN)的抑制作用,并初步探讨其机制。方法健康新西兰大耳白兔60只,雌雄不限。随机分为实验组Tet0.5g·L-1组、1g·L-1组、2g·L-1组,艾氟龙(FML,1g·L-1)阳性对照组及生理盐水阴性对照组。于PRK术后1周、2周、1个月、2个月用裂隙灯显微镜进行Haze的分级并记录,用免疫组织化学法检测角膜基质FN的表达,并应用计算机图像分析系统进行定量分析。结果不同浓度Tet、FML滴眼,术后各时间点Haze形成明显减轻,与生理盐水相比差异有显著统计学意义(P<0.01)。正常兔角膜基质FN为阴性表达;PRK术后1个月生理盐水组角膜基质细胞FN呈强阳性表达;Tet各浓度组FN呈弱阳性表达;FML组FN呈阳性表达。Tet各浓度组与FML组、生理盐水组相比差异有显著统计学意义(P<0.01)。结论Tet可能是通过减少细胞外基质——FN的生成而发挥其对Haze的抑制作用。Objective To discuss the inhibiting effects of tetrandrine(Tet) on flbronectin(FN) after photorefractive keratectomy(PRK) surgery in rabbits and explore its mechanisms. Methods Sixty rabbits were randomly distributed into five groups,Tet groups with different concentrations of 0.5 g · L^-1,1 g · L^-1 ,2 g · L^-1 ,FML group with the concentration of 1 g · L^-1 ,and normal sodium group. Haze was graded and recorded with slit lamp at 1 week, 2 weeks, 1 month and 2 months postoperatively. The expression of corneal FN was detected with immunohistochemistry,and quantitative analysis was taken with computer image analytical system. Results The Haze was obviously reduced in Tet groups and FML group compared with normal sodium group after PRK surgery under the slit lamp(P 〈 0.01 ). FN was not expressed in the normal cornea. However,FN showed the positive expression in normal sodium group at 1 month postoperatively. FN expression was weakly positive in Tet groups,and positive in FML group. Significant difference was seen in the expression of FN comparing Tet groups with FML group or normal sodium group(P 〈0. 01 ). Conclusion Tet may exert its inhibitive effect on corneal epithelium opacifleation by decreasing extracellular matrix-FN.
