干眼症患者435例临床症状分析

摘　　要：目的分析不同类型干眼症患者的临床表现及其特点。方法收集2006年10月至2008年2月就诊于我科的435例(870眼)干眼症患者的资料,并进行相关检查,包括裂隙灯检查、角膜荧光素染色、泪膜破裂时间(breakuptime,BUT)检查和基础泪液分泌试验(Schirmer Ⅰtest,SⅠt)。结果435例干眼患者中,眼部不适症状中最多见者为眼部干涩312例和视疲劳308例,其次是眼痒265例,视物模糊/视力下降247例,眼红241例,异物感213例,畏光/流泪和眼痛均为197例,眼烧灼感112例,睫毛上粘有碎屑、球结膜出血和眼睑痉挛者少见。女性发病率多于男性。非-Sjgren综合征眼患者386例,平均年龄(34.70±12.84)岁。BUT≤5s者410例;角膜荧光素染色阳性患者427例;SⅠt≤5mm者335例,6mm≤SⅠt<10mm者83例。结论干眼症患者的临床症状具有多样性特点,尤以眼干涩、视疲劳、眼痒和视物模糊/视力下降最为多见;角膜荧光素染色检查、BUT、SⅠt检查是诊断干眼症的主要依据。Objective To analyze the main clinical characteristics of different kinds of dry eye syndrome. Methods A total of 435 new outpatients （ 870 eyes ） from October,2006 to February,2008 with typical dry eye symptoms were given a questionnaire in our department and were also given some dry eye tests, including Slit-lamp examination, corneal fluoreseein staining test, tear break-up time （ BUT ） and basal Schirmer Ⅰ test （ S Ⅰ t ）. Results The most common complications were ocular dryness in 312 cases and asthenopia in 308 cases,then followed ocular itching in 265 cases,impairment of vision in 247 cases, red eye in 241 cases,foreign body sensation in 213 cases, photophobia or lacrimation in 197 cases, ophthalmalgia in 197 cases and ocular burning sensation in 112 cases. The complications of the scurf on the eyelashes, bulbar conjunctival hemorrhage and spasm of eyelid were less. Morbidity of the dry eye syndrome in the female was more than that of male. Non-Sjogren syndrome was in 38d cases. The mean age was 34.70 ± 12. 84 years old. The positive expression of corneal fluoreseein staining test was in 432 cases,410 cases with BUT≤5 seconds,335 cases with S I t≤5 mm,and 83 cases with 6 mm≤ S I t ≤ 10 mm. Conclusion The clinical characteristics of the dry eye syndrome are diverse. The most common symptoms are ocular dryness, asthenopia, ocular itching of eye and impairment of vision. According to the corneal fluorescein staining test exams, BUT and corneal point staining, or/and S I t examination, the dry eye syndrOme can be diagnosed.
