内镜下激光激发自体荧光在胃癌及其癌前病变诊断中的应用

摘　　要：目的研究内镜下激光激发自体荧光对胃癌及其癌前病变诊断的意义。方法有上消化道症状、行内镜、激光荧光和病理活检的198例患者,采用医用激光荧光快速诊断仪测定胃癌及其癌前病变的自体荧光光谱,并与组织病理结果比较。结果20例经病理检查诊断为胃癌的患者中,16例可检测到肿瘤特征峰,符合率为80.0%;22例经病理诊断为胃黏膜非典型和/或肠化患者中,19例检测到肿瘤特征峰,符合率为86.4%。156例胃溃疡及慢性浅表性胃炎患者中,141例未检测到肿瘤特征峰。自体荧光检查与病理学检查的总符合率为88.9%,对胃癌及其癌前病变的敏感性为87.7%,特异性为90.8%。结论激光荧光光谱测定对胃癌及其癌前病变的诊断具有较高的敏感性及特异性,可以作为辅助检查手段。Objective To study the value of laser-induced auto-fluoroscence on the diagnosis of gastric cancer and its precancerosis. Methods Endoscopic laser-induced fluorescence diagnosis spectra were taken in 198 patients with upper gastrointestinal symptom. Results 198 patients were diagnosed by endoscopy and biopsy, including 20 gastric cancer, 22 gastric precancerosis, 156 gastrelcoma and chronic superficial gastritis. Characteristic peaksof auto-fluorescence in cancerous tissue were observed in 16 out of 20 patients with gastric cancer and 19 out of 22patients with gastric precancerosis. The coincidence was 80.0% and 86.4% respectively. While 141 had no such peaks out of 156 patients with gastrelcoma and chronic superficial gastritis. Conclusion Laser-induced autofluorest.ence spectra have higher sensitivity and specificity in the diagnosis of gastric cancer and its precancerosis, and can be used as an effective means of supporting inspections.
