摘 要:目的:探讨咬肌EMG检测在运动神经元病(MND)诊断中的价值。方法:对30例MND患者均进行常规EMG检测,包括上肢、下肢、胸锁乳突肌及咬肌,测定自发电位,轻收缩时运动单位电位(MUP)的多相波、时限及波幅,募集相。比较患者咬肌EMG与胸锁乳突肌EMG相关性及差异。结果:咬肌EMG病变程度与胸锁乳突肌EMG比较,自发电位、运动单位电位(MUP)时限和波幅异常表现均无显著差异(P>0.05);但多相波、募集相比较有显著差异(P<0.05)。时限增宽幅度分别为66.44%、58.88%无显著差异(P>0.05)。结论:MND患者咬肌EMG检查所见异常表现与胸锁乳突肌EMG检测的异常表现具有可比性,在MND患者伴有颈椎病,尤其是患有高颈段颈椎病时,进行咬肌EMG的检查更具有鉴别诊断的积极意义。Objective: To evaluate value of EMG in diagnosis of MND. Method: EMG was examined in 30 cases with MND including the upper and lower limbs, sternocleidomastoid muscle and masseter muscle, spontaneous potential and polyphase wave, duration, amplitude of MUP and voltage and recruitment phase were recorded, and compared correlation and difference of EMG between masseter muscle and stemocleidomastoid muscle. Result: EMG in masseter muscle and sternoeleidomastoid muscle showed that there are not different in spontaneous potential and action potentials ( P 〉 0. 05 ), while the multiple -phase wave and recruitment phase was found significantly difference (p 〈 0. 05). The duration were widen by 66. 44%, and 58.88% respectively ( P 〉 0.05 ) . Conclusion: The EMG abnormalities were found comparability between masseter muscle and sternocleidomastoid muscles. EMG in masseter muscle may be helpfulin differential diagnosis of MND with spondylosis particularly with high level cervical spondylosis.
