急性坏死性胰腺炎大鼠脑髓鞘损伤的初步观察

摘　　要：目的:研究急性坏死性胰腺炎(ANP)大鼠24h内脑脊液和血清髓鞘碱性蛋白(MBP)水平的动态变化。方法:分别诱导SD大鼠ANP和急性水肿性胰腺炎(AEP)模型,同时设立假手术组。分别于模型诱导后3、6、9、12、24h抽取小脑延髓池脑脊液和腹腔动脉血液。以ELISA法测定脑脊液、血清中MBP的含量。透射电镜观察髓鞘超微结构变化。结果:ANP组大鼠诱导后3h,脑脊液MBP水平明显高于AEP组和假手术组(P<0.01),6、9h时仍维持在较高水平(P<0.05),12、24h时则降至较低水平。ANP组、AEP组和假手术组血清MBP水平无显著差异(P>0.05),在24h内维持于较低水平。电镜检查,3～9hANP组脑髓鞘主要呈轻度的损伤性改变,如空泡化、板层结构散乱等改变。结论:大鼠ANP早期存在脑组织髓鞘结构损害,表现为脑脊液MBP水平明显升高。Objective To investigate the changes of cerebrospinal fluid （CSF） and serum myelin basic protein （MBP） level in an experimental rat model with acute necrotizing pancreatitis （ANP）. Methods The model with ANP was induced in SD rats, acute edematous pancreatitis （AEP） and sham-operated rats were used as controls. CSF and blood samples were obtained 3, 6, 9, 12 and 24 hours after the introduction of experimental models, and were analyzed for MBP concentration by using an ELISA kit. The cerebral white matter was examined by transmission electron microscope. Results The kinetic CSF MBP concentration of ANP rats showed a rapid uprise at 3 h （P〈0.01）, and remained at a high level at 6 and 9 h after introduction （P〈0.05）, thus showing a significant difference compared with the levels observed in the AEP or sham-operated rats; then followed by a slow decline after 12 h （P〉0.05）. The concentrations of serum MBP in the ANP, AEP and sham-operated rats did not show any significant differences and remained at lower levels. In the ANP group, slight or focal chaos, such as vacuolation and debunehing, was observed in the myelin sheaths by electron microscopy at 3 h and 9 h. Conclusions The experimental results provide evidence that demyelination oecursing in the early stage of ANP. High concentration of MBP in the CSF reflects the demyelination.
