伊立替康联合氟尿嘧啶、亚叶酸钙治疗晚期结直肠癌

摘　　要：目的：观察和评价伊立替康（CPT-11）联合氟尿嘧啶（5-FU）与亚叶酸钙（cF）治疗晚期结直肠癌的疗效及毒副反应。方法：45例晚期结直肠癌行CPT-11180mg／m2，静脉滴注90min，第1天，CF200mg／m2，静脉滴注2h，第1-2天，5-FU400mg／m2，CF滴完后静推，然后5-FU600mg／m2，静滴22h，第1～2天，每2w1周期。连用4周期评价疗效。结果：45例患者有效率（CR＋PR）33．33％，SD42．22％，PD24．44％。中位生存期（MST）12．8个月，中位疾病进展时间（TTP）7．0个月。主要毒副反应为中性粒细胞减少、迟发性腹泻和胆碱能综合征。结论：CPT-11联合5-FU／CF治疗晚期结直肠癌疗效肯定，毒副反应可耐受。Objective： To evaluate the efficacy and adverse effects of irinotecan （ CPT - 11 ） combined with 5 - FU/CF for treating advanced colo- rectal cancer. Methods ：45 cases with advanced colorectal cancer were treated with CPT - 11 180mg/m2 iv dl, CF 200 mg/m2, iv dl - 2,5 -FU 400 mg/m2 iv d1 - 2,5 - FU 600 mg/m2 iv dl - 2, every two weeks was a cycle. All patients received two cycles chemotherapy. Results： In 45 pa- tients,33.3% of response rate was observed and SD was 42. 22%, PD was 24.44%. MST was 12. 8 months, TrP was 7.0 months. Major toxic effects were neutropenia and delayed diarrhea and cholinergic syndrome. Conclusion： CPT - 11 combined with 5 - FU/CF for treating patients with advanced colorectal cancer is an active regimen. Its side effects are tolerable.
