摘 要:目的:探讨谷胱甘肽S-转移酶(GST-π)在幽门螺杆菌(HP)感染胃黏膜组织中的变化及其在胃癌发生发展中的意义。方法:内镜及活检病理证实的198例各类慢性胃疾病分别为慢性胃炎组118例(组1)、肠化及异型增生组46例(组2)和胃癌组34例(组3),分别用13C呼气实验及改良Giemsa染色检测HP,S-P免疫组化检测GST-π,比较各组及组内HP阳性和阴性胃黏膜组织GST-π的表达。结果:3组GST-π阳性率分别为57.63%、73.91%和82.35%,组3较组1、组2显著升高(P<0.05)。HP阳性与HP阴性GST-π的阳性率分别是组1为44.93%与75.51%(P<0.01)、组2为55%与88.46%(P<0.05)和组3为78.95%与86.67%(P>0.05)。结论:HP感染可降低胃黏膜GST-π表达,削弱其对胃黏膜损伤的保护作用,促进了胃内致癌物的形成及致突变作用。Objective:To observe the expression of glutathione S-transferaseπ (GST-π) in gastric mucosa of patients with various gastric diseases and to investigate the relationship between expression of GST-π and virulence of Helicobacter Pylori(HP)in the development of gastric carcinoma.Methods:One hundred and ninety eight patients with various chronic gastroduodenal diseases confirmed by endoscope and pathology were included in this study.HP infection was confirmed by ^13C urease breath test and histological examination of biopsy specimens with Giemsa dyeing,GST-π was detected by S-P immunochemiohistology.The patients were divided into three groups as Group of chronic gastritis (group 1),Group of intestinal metaplasia and dysplasia(group 2)and Group of gastric carcinoma(group 3).Results:The average positive rate of GST-π expression in the three groups were 57.63%,73.91% and 82.35% respectively and the GST-π positive rate of group 1 and group 2 were significantly lower than that of group 3 (P〈0.05).We analyzed the positive rates of GST-π expression in all 3 groups.There were significant differences between HP positive and HP negative cases.The positive rate of GST-π expression was significantly lower in the HP positive patients than that in HP negative cases in group 1(44.93% vs 75.51%,P〈0.01)and in group 2(55% vs 88.46%,P〈0.05),but no significant difference in group 3 (78.95% vs 86.67%,P〉0.05).Conclnsion:HP infection may decrease expression of GST-π and its defensive function,which is highly correlated with an increased risk of cancer.
