支气管哮喘协会会员哮喘病情控制情况的调查

摘　　要：目的调查北京大学人民医院支气管哮喘协会会员哮喘病情控制情况,比较并评估患者自我评价、哮喘控制测试(ACT)问卷评分及呼气峰流速(PEF)测定结果。方法选取2007年9月29日前来参加哮喘协会活动的会员,首先由会员本人对其病情进行自我评价,而后填写ACT问卷和进行PEF测定,并对会员糖皮质激素的使用情况进行调查。结果参加测试的患者76例,平均年龄为(60.6±11.5)岁。自我评价中,达到稳定或偶尔发作的比例为89.5%(62/76);ACT问卷结果达到良好控制以上水平的为75%(48/64);PEF测定中PEF测定值处在绿区的占50.8%(31/61),PEF测定值处在黄区的占36.1%(22/61)。自我评价结果与ACT评价结果符合的占测试患者的40.6%,自我评价高于ACT的占56.3%。PEF值与ACT评价结果比较,绿区组27例,ACT分值平均为(21.33±3.44)分;黄区组17例,ACT分值平均为(21.12±3.24)分;红区组7例,ACT分值平均为(17.43±4.47)分。绿区组的ACT分值显著高于红区组,绿区组的ACT分值与黄区组比较,以及黄区组ACT分值与红区组比较差异均无统计学意义。规律使用吸入糖皮质激素的比例为95.6%(65/68)。结论接受北京大学人民医院"三位一体"教育管理模式的会员大多数哮喘病情能达到良好控制以上的水平;如以ACT问卷结合PEF测定,更能准确评估和监测哮喘控制情况;建立医患之间的伙伴关系是提高哮喘控制水平和用药依从性的有力保障。Objective To investigate the control level in asthmatic patients who were members of asthma association in the People＇s Hospital of Peking University. Methods 76 asthmatic patients attended the third meeting of asthma association and asthma test was conducted. The patients self-evaluated the state of illness, filled the questionnaire of asthma control test （ACT） while peak expiratory flow （PEF） was measured, The steroids usage was also investigated. Results The average age of the patient investigated was （60. 6 ± 11.5 ）years old. The percentage of well control by self-evaluatlon was 89. 5% （62/76） ,75 % （48/64） by ACT questionnaire. 50. 8 % （ 31/61 ） of the patients fell in green region and 36. 1% （ 22/61 ） fell in yellow region of PEF. The coincidence rate of self-evaluation with ACT was 40. 6%. ACT scores cgmparable to green reglon,yellow region and red region were 21.33 ± 3.44,21.12 ± 3.24 and 17.43 ± 4. 47 ,respectively. There were no significant difference of ACT score between green region group and yellow region group, or yellow region group and red region group. Conclusion Most of asthma association members could achieve well control which could be ascribed to asthma education and management program on the basis of partner relationship between doctors and patients.
