ECF与DCF方案治疗晚期胃癌效果比较

摘　　要：目的比较ECF与DCF方案治疗晚期胃癌的近期疗效和不良反应。方法将45例经病理检查确诊的晚期胃癌病人分为2组。ECF组21例,给予表阿霉素60 mg/m^2静脉注射,第1天;顺铂75 mg/m^2分3-4 d静脉滴注;氟尿嘧啶每天500 mg/m^2持续输注96 h。DCF组24例,给予多烯紫杉醇75 mg/m2静脉滴注2 h,第1天;顺铂与氟尿嘧啶用法同ECF组。每3周为1个周期,2-3个周期评价疗效,对两组缓解率、生活质量改善率及不良反应进行比较。结果ECF组与DCF组的缓解率分别为56.2%和58.3%,两组差异无显著性（P〉0.05）;两组神经系统毒性反应有明显差异（P=0.001）。结论ECF和DCF方案对晚期胃癌病人近期疗效均较好,不良反应可以耐受,生活质量明显提高。Objective To compare the efficacy and toxicity between ECF regimen （epirubicin--Cisplatinq-5-FU） and DCF regimen （docetaxel＋Cisplatin-F 5-FU） for advanced gastric carcinoma. Methods Forty-five cases of advanced gastric carci- noma were divided into two groups. ECF group consisted of 21 patients who received epirubicin 60 mg/m^2 iv injection on the first day. DCF group consisted of 24 patients receiving 96 h iv drip of 75 mg/m^2 docetaxel on the first day. Both groups were given iv drip of 75 mg/m^2 Cisplatin and 500 mg/m^ 5-FU within 3-4 days. Both regimens were repeated every three weeks. After 2--3 cycles of treatment, the remission rate, quality of life and side effects were compared. Results The overall remission rates of the ECF group and DCF group were 56.2%and 58.3%, respectively （P〉0.05）. But there were significant differences in peripheral neuropathy between the two groups （P=0. 001）. Conclusion Both ECF and DCF regimens showed a good short-term effect for advanced gastric carcinoma, the side effects are accepted, the quality of life improved.
