摘　　要：目的探讨MRI对子宫恶性肿瘤的MRI诊断及分期价值。方法回顾性分析50例经病理证实的子宫恶性肿瘤患者的MRI表现、诊断及分期,包括宫颈癌38例和12例子宫内膜癌。将MRI诊断结果与术后病理诊断比较。结果①子宫增大,子宫颈软组织肿块,增强病灶明显不均匀强化,肿块内有低密度坏死灶并与周围组织分界不清是宫颈癌MRI表现特点;子宫内膜癌的MRI的典型表现为子宫内膜增宽,内膜腔扩大,在高信号的内膜腔中混杂有结节状的中等或低信号区。②在T2WI中,子宫肿瘤组织通常呈不均匀中等信号与正常子宫组织及宫旁结构之间具有良好的对比,对判断肿瘤大小及侵犯范围价值最大。③MRI对宫颈癌分期诊断准确率为86.84%,子宫内膜癌分期准确率为91.67%。结论MRI能准确显示子宫肿瘤病灶,并多方位观察癌肿侵犯范围,分期客观准确,对临床制订治疗方案及术后评估具有重要指导意义。Objective To approach the diagnostic and staging value to uterus malignant tumor by MRI. Methods The 50 cases＇ MRI display,diagnosis,and staging of uterus malignant tumor including 38 cases of cervix carcinoma and 12 cases of endometrial carcinoma which confirmed by pathology were retrospective analyzed, and the diagnostic results of MRI were compared with ＂the pathological.Results （1）There were features on the uterus carcinoma＇s MRI display,including hysterauxesis,lump in the cervix soft tissue,obviously uneven enrichment on the enhance focus,the necrosis focus with low density shadow in the lump and the lump with no clear boundary;Tbe widen endometria,dilated cndometrial cavity,and the high signal mixed with nodous moderately or mild signal field in the endometrial cavity were the typical MRI display of the endometria carcinoma. （2）The cancer tissue with uneven moderately signal usually in the T2WI photograph had satisfactory contradistinction to the normal uterus tissue and lateral structure,and it had greatest value to the deeision of the tumor size and invasive extent. （3）The accuracy rate were 86.84% to the cervix carcinoma＇s staging diagnosis by MRI,and 91.67% to the endometrial carcinoma＇s. Concslusion The focus of uterus tumor would be accurately displayed by MRI,and the invasire extent be observed front multidirection.The staging was accurate and objective, and had important direct significance to the clinical therapeutic plan and postoperative evaluation.
