趋化因子和CD_(34)^+干细胞联合移植对梗死后大鼠左室重塑影响的实验研究

摘  要:目的探讨趋化因子SDF-1联合同种异体CD34+干细胞移植对心肌梗死后大鼠左室重塑的影响。方法Wistar雄性大鼠38只,随机分成3组:SDF-1联合干细胞移植组(13只);急性心肌梗死对照组(13只);假手术组(12只)。移植组和对照组结扎左冠状动脉前降支造成急性心肌梗死模型。另选同种异体大鼠13只,粒细胞集落刺激因子(G-CSF)动员后免疫磁珠法分离外周血CD34+干细胞,在梗死模型制作后7d,经静脉联合趋化因子SDF-1注入到移植组大鼠体内,对照组和假手术组注入等量磷酸盐缓冲液(PBS)。分别于心梗术前、干细胞移植术后1周和4周行心脏超声检查,术后4周检测心肌组织血管紧张素Ⅱ(AngⅡ)和胶原含量。结果心肌梗死后4周,与对照组比较,移植组左室射血分数(LVEF)、左室短轴缩短率(FS)明显提高(P<0.01),左室收缩末直径(LVESD)、左室舒张末直径(LVEDD)明显减小(P<0.01);移植组心肌组织AngⅡ、胶原含量也有不同程度下降(P<0.05)。结论经静脉联合移植趋化因子SDF-1和外周血干细胞是安全可行的,它能有效地改善心功能,抑制心室重塑,为造血干细胞在心血管疾病中的应用提供理论依据。[Objective] To investigate the effects of transplantation chemotatic factor SDF-1 and CD34^- positive stem cells on cardiac function and myocardial remodeling in the infraeted rats, and to evaluate the efficacy and safety of stem cell transplantation. [Methods] Thlrty-eight male Wister rats were randomized into SDF-1 and cell infusion group(n=13), control group(n=13) and sham-operated group(n=12). AMI was induced by ligation of left anterior descending coronary artery. CD34^+ stem cells mobilized with granulocyte-colony stimulating faetor(G-CSF) were purified by immunomagnetic beads in donor rats. Seven days after the injury ,SDF-1 and CD34^- positive stem cells were infused through the veins of reeipent rats. Cardiac function was evaluated before MI, 1 w and 4 w after cell delivery. Concentrations of angiotensin Ⅱ and collagen were detected 4 w later. [Results] Compared with control group, left ventrieular ejection fraction(LVEF), fractional shortening (FS) were improved significantly (P 〈0.01), whereas left ven-tricular end systolic diameter (LVESD), left ventricular end diastolic diameter (LVEDD) were lower (P〈0.01) in SDF-1 and cell infusion group. The concentrations of angiotensin Ⅱ and collagen, and ratio of collagen type I and Ⅲ in the non-infarct area in cell infusion group were markedly lower than those in the control group (P〈0.05). [Conclusion] Introvenous delivery of SDF-1 and Hematopoietic stem cells (HSC), which offers the theoretical evidence for the further study of cardiovascular disease ,can significantly improve the cardiac function, lessen the degree of myocardial remodeling and it is a safe and effective ahemation for the treatment of AMI.
