摘 要:目的:观察结直肠癌术后化疗的骨髓抑制毒性反应与贞芪扶正颗粒的解毒保护作用。方法:采用随机分组对照法,回顾性调查接受FOLFOX或FOLFIRI联合化疗的结直肠癌术后患者的血白细胞、血红蛋白浓度、血小板平均体积和血小板数量,154例分为FOLFOX、FOLFOX+贞芪扶正颗粒、FOLFIRI、FOLFIRI+贞芪扶正颗粒和对照等5组,化疗均为1疗程/月,共6疗程。第1次化疗后1月开始监测各组的血白细胞计数、血红蛋白浓度与血小板数量,1次/月,至化疗结束后3个月,共9次。结果:比较对照组,FOLFOX和FOLFIRI组的血白细胞计数、血红蛋白浓度、血小板数量和平均体积均明显下降,并随化疗次数增加而下降,化疗结束后3月恢复至正常。加贞芪扶正颗粒的两组白细胞数、血红蛋白浓度、血小板数量和平均体积下降幅度则明显小于无贞芪扶正颗粒组。结论:结直肠癌术后常见的FOLFOX或FOLFIRI方案联合化疗均可导致三系骨髓抑制,随化疗次数增加而加重。贞芪扶正颗粒有不同程度的解毒保护作用。Objective To examine the haemotopoietic toxicity of FOLFOX/FOLOFIRI chemotherapy after colorectal operation and the preventive and therapeutic efficacy of Zhenqifuzheng(贞芪扶正)granules on the toxicity. Method One hundred and 22 patients were divided into four groups randomly and given standard FOLFOX or FOLOFIRU chemotherapy in conventional doses, for 6 courses within 6 months; Of them, either of the FOLFOX groups received Zhenqifuzheng therapy(in conventional dose for two weeks) simultaneously; so did one of the FOLOFIRI groups. The four groups and the control without chemotherapy and Zhenqifuzheng therapy were observed in the counts and mean volume of the plastocytes, the leucocyte counts (WBC) and the haemoglobin (HB) level; the data were synthesized by SPSS10.0 software statistically. Results Both groups with Zhenqifuzheng displayed a slighter haematopoietic inhibition than those without Zhenqifuzheng significantly but still a heavier inhibition compared with the control according to the WHO standard. Furthermore, of the four chemotherapy groups, all the monitored items including the counts and mean volume of the plastceytes, WBC and HB, decreased as the chemotherapy coursed increased( P 〈 0.01), and recovered up to the common level three months after the chemotherapy was finished( P 〈 0.05) except the FOLFOX group. Those of the groups with Zhenqifuzheng therapy decreased in a less degree ( P 〈 0. 01 ). Conclusion FOLFOX/FOLOFIRI chemotherapy after colorectal operation can cause tri - strain haematopoietic depression at a course - dependent correlation. Zhenqifuzheng therapy has a therapeutic and preventive efficacy on the toxicity.
