下咽癌的超声影像表现及其与CT对照

摘　　要：目的探讨下咽癌的超声影像特点。方法使用GELogiq9型超声诊断仪分析14例下咽癌声像图,并与CT对照。结果14例下咽癌均完整显示,其中形态不规则11例,类圆形3例;边界均欠清楚;低回声型7例,低回声伴内侧偏强回声型4例,伴中央强回声型3例;内部密布动脉血流12例,少许短棒状动脉血流2例;与CT对照,超声和CT均能完整显示下咽癌病变;对颈部软组织、喉内(软骨)受侵,均判断正确。超声判断喉内真假声带侵犯假阳性3例,假阴性3例;CT判断喉内真假声带侵犯假阳性3例,判断甲状软骨侵犯假阴性1例。结论超声影像诊断和CT一样,显示下咽癌病灶的整体性较好,特别是梨状窝癌,还可对甲状软骨、颈部软组织、喉内(软骨)的受侵作出判断。判断喉内真假声带受侵尚不满意。Objective To approach the ultrasonographic features of hypopharyngeal cancer. Methods We analyzed the US imaging of 14 patients diagnosed with hypopharyngeal cancer, and compared US with CT. Results All the hypopharyngeal carcinomas were completely shown in the US imaging in the 14 patients, of which 11 lesions were in irregular shape and 3 were in similarly round. All the lesions had indistinct margin. 7 were hypoechoic and other 7 were hyperechoic mixed, of which 4 located in the edge and 3 in the centre. Twelve carcinoma lesions had arterious vaseularity, and 2 had less vaseularity with some short rod shape vessels. Compared with each other, both CT and US could show complete hypopharyngeal cancer lesions ,and make correct assessment for the neck soft tissue and larynx cartilage infiltration. As to the true and false vocal cord extension, US had 3 false positive and 3 false negative, while CT had 3 false positive and 1 false negative, in which thyroid cartilage infiltration was missed. Conclusions US had the ability to show the entirety of hypopharyngeal cancer as CT, especially for the pyriform sinus carcinomas, and could make assessment for the neck soft tissue and larynx cartilage infiltration, but still had unsatisfactory judgement for the true or false infiltration of vocal cords.
