摘 要:[目的]了解亚甲基四氢叶酸还原酶基因(MTHFR基因)的表达与食管癌发生的关系,探讨食管癌相关环境危险因素与MTHFR基因表达水平之间的交互作用。[方法]收集97例淮安地区食管癌新发病例的食管癌组织及远端癌旁组织,采用实时荧光定量RT-PCR方法定量分析食管癌组织与癌旁组织中MTHFR基因的表达水平,同时运用单纯病例研究分析MTHFRmRNA水平与环境危险因素之间的交互作用。[结果]MTHFRmRNA表达水平在食管癌组织显著低于癌旁组织,与癌旁组织相比,癌组织的MTHFRmRNA表达水平下调了48%,条件Logistic回归分析表明MTHFRmRNA表达下调与食管组织癌变有统计学关联(OR=1.20,95%CI:1.05~1.37);饮茶习惯与MTHFRmRNA低表达之间存在拮抗作用,且有剂量-反应关系。饮酒与MTHFRmRNA低表达之间的协同作用无统计学意义。[结论] MTHFRmRNA表达水平可反映食管组织叶酸代谢的能力,其下调与食管癌发生有关;饮茶可拮抗MTHFRmRNA表达缺陷对食管癌发病的影响。[ Objective ] To determine whether the mRNA expression of MTHFR ( methylenetetrahydrofolate reductase ) gone is associated with the risk for ESCC ( esophageal squamous cell carcinoma ) and the interaction between environmental risk factors and expression level of MTHFR mRNA. [ Methods ] 97 patients with newly diagnosed, untreated ESCC were recruited in Huai' an city. Esophageal cancer tissues and tissues adjacent to the tumors were collected to determine the mRNA levels of MTHFR gone by quantitative real-time reverse transeription-PCR using SYBR: Green I with β-actin as an internal control. Case-only study was used to analyze the gone-environmental interaction at a significance level of 5%. [ Results ] Compared with tissues adjacent to the tumors, the relative expression level of MTHFR was significantly reduced in esophageal cancer tissues. The conditional logistic regression analysis showed that the increased risk for esophageal cancer were significantly associated with aberrant expression of MTHFR ( OR=1.20, 95%CI: 1.051-1.371 ). The ease-only study showed that the interaction were statistically significant between tea-consumption and reduced expression of MTHFR, which demonstrated a dose-response relationship. The interaction between alcohol drinking and reduced expression of MTHFR gene showed no statistical significance. [ Conclusion ] Reduced expression of MTHFR was significantly associated with the risk for ESCC, suggesting aberrant capacity of folate metabolism in Huai' an population. Tea-consumption showed a significant effect on suppressing the risk for ESCC associated with the reduced MTHFR expression.
