甲酰四氢叶酸钙漱口液对造血干细胞移植后患者口腔黏膜炎的疗效观察

摘　　要：【目的】观察甲酰四氢叶酸钙(CF)漱口液对血液系统恶性肿瘤患者异基因造血干细胞移植后口腔黏膜炎(OM)的疗效。【方法】58例行异基因外周血造血干细胞移植的血液系统恶性肿瘤患者随机分为甲酰治疗组和对照组,治疗组除应用一般的漱口液(甲硝唑、洗必泰等)以外,于移植后1d(记为+1d)起加用含CF的漱口液含漱,对照组主要应用一般的漱口液治疗。【结果】甲酰组OM的发生率为76.6%,对照组则为92.8%,较甲酰组高16.2%。甲酰组轻度(Ⅰ～Ⅱ度)OM的发生率为63.3%,对照组为46.4%,较甲酰组低16.9%。甲酰组重度(Ⅲ～Ⅳ度)OM的发生率为13.3%,对照组为46.4%,较甲酰组高33.1%。两组相比差异均有显著性(均P<0.01)。甲酰组OM开始发生的时间平均为移植后3.2d(即+3.2d),对照组平均为+3.0d,两组差异无显著性(P>0.05)。甲酰组OM愈合的时间平均为+11.7d,对照组平均为+23.2d,较甲酰组延长11.5d。甲酰组OM的持续天数为8.4d,对照组为20.3d,较甲酰组延长11.9d。以上统计差异均有显著性(P<0.01)。【结论】CF漱口液对血液系统恶性肿瘤患者异基因造血干细胞移植后口腔黏膜炎的预防和疗效确切,无不良反应,耐受性好。[ObjectivelTo investigate the effect of calcium formyltetrahydrofolate（CF） mouthwash on oral mucositis（OM） of patients with hematological malignancies after allogeneic hematopoietie stem cell transplantation. [Methods] Fifty eight patients after allogeneic peripheral blood stem cell transplantation were divided into CF group and the control group randomly. Besides the general application of the mouthwash （metronidazole, chlorhexidine, etc. ） just like in the control group, patients in treatment group were treated also with the CF gargle mouthwash solution from the 1st day after transplantation. [Results] The incidence of OM in CF group and the control group were 76.6% and 92.8% respectively. The incidence of mild （Ⅰ～Ⅱ degrees） and severe （Ⅲ or Ⅳ degrees） OM in the CF group were 63.3% and 13.3% , while the incidence were 46.4% and 46.4% in the control group. The above differences were statistically significant （ P 〈0.01）. Although the difference in the time at the beginning of OM was not statistically significant between the CF group（3.2d） and the control group （3d）, but the difference in the healing time was significant （11. 7d vs. 23. 2d）. And there was significant difference in the suffering time between CF group and control group （8.4d vs. 20.3d）. [Conclusion] The CF mouthwash has certain effect on the prevention and treatment of oral mueositis in patients with hematological malignancies after ailogeneic hematopoietic stem cell transplantation. It is of well tol eration and is of no adverse reaction.
