439例大肠癌血清CEA水平的临床分析

摘　　要：【目的】探讨血清癌胚抗原(CEA)与大肠癌的关系。【方法】采用化学发光法测定了439例大肠癌CEA水平,并进行相关分析。【结果】血清CEA的阳性率与年龄、性别、肿瘤所处的肠段位置以及肿瘤转移的部位均无显著相关性(P>0.05),而与病理分期(Dukes分期)有密切关系(P<0.01)。在各期大肠癌中,Dukes D期CEA的阳性率远高于其它各期者,达到95.1%;其次是Dukes C期,阳性率为36.4%,Dukes B期CEA的阳性率仅为2.3%,Dukes A期CEA的阳性率为0。【结论】CEA对于早期大肠癌无诊断价值;但对于大肠癌远处转移或腹腔转移的敏感度非常高,对于协助诊断大肠癌远处脏器或腹腔转移有积极的意义。[Objective] To investigate the relationship between serum carcinoemhryonic antigen（CEA） and colorectal cancer. [Methods]Serum CEA level of 439 cases with colorectal cancer was detected by chemiluminescence method, and the correlation analysis was made. [Results] The positive rate of serum CEA was not associated with age, sex, the tumor＇s position in intestine and the site of tumor metastasis（ P〉0.05）. The positive rate of serum CEA was correlated to the pathological stages（Dukes staging）（ P〈0.01）. The positive rate of serum CEA in stage Dukes D was far higher than that in the other stages, reaching to 95.1%. The positive rate of serum CEA in stage Dukes C, stage Dukes B and stage Dukes A was 36.4%, 2.3% and 0, respectively. [Conclusion]Serum CEA has no value for the early diagnosis of colorectal cancer, but is very highly sensitive and bas positive significance for the diagnosis of colorectal cancer metastasis or peritoneal metastasis to distant organs.
