巨大肾癌术前肾动脉双重栓塞的临床应用

摘　　要：目的探讨巨大肾癌术前肾动脉双重栓塞的疗效。方法7例中晚期巨大肾癌术前运用无水酒精与明胶海绵进行肾动脉双重栓塞。结果全部病例栓塞成功,并在随后的1～5d成功地进行了肾癌根治性切除。术中见肾周组织水肿,脂肪液化,容易分离;肾动脉搏动明显减弱或消失,出血量明显减少。结论巨大肾癌术前肾动脉双重栓塞能有效地提高手术的安全性与成功率,为部分中晚期肾癌患者提供了的一种安全、有效的治疗手段。Objective To investigate the efficacy of preoperative renal artery double embolization in giant renal carcinoma. Methods Double embolization was performed by using of absolute alcohol and gelatin sponge in 7 cases with mid-terminal stage of giant renal carcinoma preoperatively. Results All the cases were obtained embolization successfully, with the radical nephrectomy 1-5 days after treatment. The perinephric edema could be found during operation, with liquefaction of fatty tissue easy to separate. The pulsation of renal artery weakened or disappeared prominently, with less bleeding. Conclusion Preoperative double renal artery embolism in giant renal carcinoma could increase the security and success rate of operation. It could offer a kind of safe and effective therapeutic method for part of patients with meal-terminal stage renal cell carcinoma.
