摘 要:目的评价全身磁共振扩散加权背景抑制成像(WBMR-DWIBS)探查恶性肿瘤全身转移的价值。资料与方法10名健康志愿者及46例恶性肿瘤患者行WBMR-DWIBS检查。对比分析WBMR-DWIBS图像、常规MR图像和CT图像。结果WBMR-DWIBS检出46例恶性肿瘤患者原发灶以外全身转移瘤125处,检出率为90%(125/139),对肝脏、骨骼和淋巴结转移的检出率明显高于肺和脑转移(χ2=30,P<0.001)。WBMR-DWIBS诊断转移瘤假阴性14处,假阳性16处,假阳性主要包括肝脏及椎体良性病变;呼吸、心脏大血管搏动伪影,脑组织背景信号高及成骨性转移是导致相应部位假阴性诊断的主要原因。结论WBMR-DWIBS对转移瘤具有较高的检出率。Objective To evaluate the value of whole-body magnetic resonance diffusion weighted imaging with background signal suppression(WBMR-DWIBS) in the detection of metastasis. Materials and Methods 10 healthy volunteers and 46 patients with malignant tumors underwent WBMR-DWIBS. WBMR-DWIBS images, conventional MR images and CT images were compared. Results 125 metastatic lesions were found in 46 patients with malignant tumors on WB- MR-DWIBS images with the sensitivity of 90%. The detection rate of metastases in liver, bone and lymph nodes was significantly higher than that in lung and brain (P 〈 0. 001 ). 14 cases were false negative and 16 patients were false positive. The false positive resulted from benign lesions in liver and vertebral body. Respiratory artifact, cardiac motion artifact, high background signal of brain and osteoblastic lesions caused the false negative. Conclusion WBMR-DWIBS could be a sensitive method in the detection of metastatic lesions.
