摘  要:目的探讨体成分与肺通气功能的相关性。方法根据自愿参加的原则,以马鞍山市某中学初一、初二两个年级33名超重和肥胖的学生为研究对象,进行肺功能测试获得通气功能指标,体检获得身高、体重、皮脂厚度、腰围、臀围等各项身体测量指际。不同体脂水平、通气功能指标变化的男女比较及不同肥胖程度的比较采用 t检验,多元 logistic 回归分析体成分与通气功能的相关性。结果男生的身高、体重、瘦体重、腰围、腰臀比、肺活量、肺活量指数、最大通气量、第1秒用力呼出量和单位通气量的均数都大于女生。肥胖组身高、体重、BMI、三处皮脂厚度、体脂百分含量、瘦体重、臀围、最大通气量、1秒率的均数都大于超重组。多元logistic 回归分析结果表明,肺活量指数的升高和单位通气量的减少预示了肥胖危险的增加。结论身体脂肪过度积聚可引起通气功能和呼吸调节的改变,呼吸系统功能下降与肥胖具有极为显著的关联。[临床儿科杂志,2008,26(10):875-878]Objectives To explore the effects of body composition on pulmonary ventilation function among adolescents. Methods Thirty-four middle school students were recruited. Pulmonary ventilation function was tested. Height, weight, and triceps, subscapular, abdominal skinfold were measured hy standard procedures. The differences of body fat and pulmonary ventilation function between boy and girl, obesity and overweight, were analyzed by student t test. The stepwise muhiple regression models were constructed to examine the relationship of body composition and pulmonary ventilation function. Results The means of height, weight, lean body weight, waist circumference, waist and hip ratio, forced vital capacity(FVC), vital capacity index(FVC/weight) , maximal voluntary ventilation (MVV), force expiratory volume in ls and MVV per body area(MVV/BSA)were all higher in boys than those in girls. The means of height, weight, BMI, skinfold thickness, body fat percent, lean body, hip circumference, maximal voluntary ventilation and force expiratory voluntary ventilation were all higher in obesity children than those in overweight children. Multinomial logistic analysis indicates that the increase of FVC/weight and the decrease of MVV/ BSA foreshow the higher risk of obesity. Conclusions The excessive aecumulation of body fat may lead the change of puhnonary ventilation function and respiratory controlling. There is an obvious association between obesity and respiratory function decrease.
