摘  要:目的:对比最大密度投影(MIP)、单纯薄层(TS)和两者结合3种阅片方式对肺癌低剂量CT普查小结节检出的差异。方法:以TS、MIP、MIP+TS阅读连续100例低剂量CT肺癌普查者图像,分别记录肺结节的总数,并且根据密度(实性、部分实性、非实性)、大小(≤4mm,>4mm)、部位(紧靠血管、远离血管胸膜、紧贴胸膜)进行分类比较。结果:MIP发现结节总数明显多于TS(85:38,P=6.8136E-9);前者发现实性(79∶28,P=3.5279E-11)及部分实性结节多于后者(4∶3),非实性结节少于后者(2∶7);前者对>4mm和≤4mm结节的优势均明显(21∶13与57∶15,P<0.05);前者对不同部位结节的检出率均能提高,其中紧靠血管结节(17∶5,P=0.0113)与非紧靠血管胸膜结节(54∶22,P=0.0162),均高于贴胸膜结节(14∶11);MIP+TS发现的结节比后两者多。结论:MIP+TS可以提高低剂量CT肺癌普查小结节的检出率。Objective:To compare thin slices (TS) ,maximum intensity projection (MIP) and MIP+TS in the detection of small pulmonary nodules in low-dose CT screening for lung cancer. Methods:The CT images of consecutive 100 people participating in low-dose CT screening for lung cancer were read by MIP,TS and MIP+TS respectively (time gap one week). The total numbers of nodules found by the three reading styles were recorded and categorized according to different density (solid, part-solid, non-solid), location (juxta vascular, non- pleural or vessel contacting, juxta-pleural) and size (≤4mm,〉4mm). Results:More nodules were found by MIP than TS (85:38,P=6. 8136E-9) ,more solid nodules (79:28, P=3. 5279E 11) and part-solid nodules (4: 3) were found by MIP,less non-solid nodules were found by MIP (2: 7);The nodule detection of different sizes were all improved markedly by MIP ( ≥4mm.. 21 : 13, P= 0. 026857 ; 〈4mm: 57 : 15, P= 7. 3025E-8);MIP increased the nodule detection rates in different locations, more nodules of jaxtavascular (17:5, P= 0.0113) and non pleuralor vessel contacting (54:22,P 0.0162) were found than those ofjaxta- pleural (14:11);more nodules were found by MIP+TS than by TS or MIP. Conclusion: More nodules can be found by TS+ MIP reading style in low-dose CT screening for lung cancer.
