中老年人颈动脉狭窄支架置入术的临床观察

摘　　要：目的：总结中老年人颈动脉狭窄支架置入术预防脑梗死和改善临床症状的效果。方法：27例患者为年龄51～76岁的中老年人，颈动脉狭窄率50％～70％7例，70％～80％19例，80％～95％1例；狭窄长度1．0～2．0cm14例，2．1～3．0cm12例，3．1～3．5cm1例。在脑保护装置下施行颈动脉支架置入术，分析颈动脉支架置入术的有效性和安全性。结果：27例在脑保护装置下颈动脉支架置入术均成功实施，残余狭窄率20％3例，10％以下24例25处。4例球囊扩张时出现一过性意识障碍，4例出现暂时性心率减慢，3例出现血压下降。临床随访时间为6～18个月，平均8．3个月，没有因为CAS而死亡及神经系统并发症发生。结论：颈动脉狭窄支架置入术是预防中老年人脑栓塞的有效手段，具有创伤小、痛苦轻、成功率高、并发症少等优点，但远期疗效还需要进一步追踪随访。Objective：To sum up the carotid artery stenosis stenting in the elderly to prevent cerebral infarction and improve the clinical symptoms. Methods：Of the 27 cases of patients aged 51 to 76 years suffering from carotid artery stenosis,7 cases had a stenosis rate of 50 percent to 70 percent,19 cases 70 percent to 80 percent,1 case 80% to 95%. In narrow length, 14 cases were 1.0 - 2.0cm, 12 cases 2. l - 3.0cm, 1 case 3.1 - 3.5cm. With the protection device in the brain, carotid artery stenting was performed, the effectiveness and safety of carotid stenting were assessed. Results： The carotid artery stenting in 27 cases were successfully implemented,the remnant narrow rate being 20 percent in 3 cases,being 10 percent in 24 cases with 25 stenosis. Four cases had transient disturbance of consciousness when the balloon expanded,four cases had a temporary reduced heart rate,3 cases had a lowered blood pressure. Clinical follow-up time was 6 to 18 months,an average of 8.3 months. There were no death due to the artery stenting and complication in the nervous system. Conclusion：Carotid artery stenosis stenting is an effective means to prevent cerebral embolism in the elderly, with minor trauma,less suffering, high success rate, fewer complications and other advantages,but longterm results need further follow-up track.
