乳腺癌超声声像图表现与免疫组化指标的关系及意义

摘　　要：目的探讨乳腺癌超声声像图表现与免疫组化指标的关系及意义。方法对70例原发性乳腺癌患者术前行超声检查,术后标本进行免疫组化测定,并将二者的结果进行相关性研究。结果①有毛刺征的肿块雌激素受体(ER)和孕激素受体(PR)阳性表达率高于无毛刺征者,但差异无统计学意义;而其原癌基因(C-erbB-2)、抑癌基因(P53)以及Ki-67抗原的阳性表达率均低于无毛刺征者,且P53、Ki-67的差异具有统计学意义。②肿块内部回声的均匀性与ER、PR、C-erbB-2、P53以及Ki-67阳性表达率差异均无统计学意义;③内部伴有微小钙化的肿块ER、PR阳性表达率低于内部无微小钙化灶者,且差异有统计学意义,而C-erbB-2、Ki-67以及P53的阳性表达率均高于内部无微小钙化灶者,但差异无统计学意义。结论乳腺癌的超声声像图与各免疫组化指标有一定的相关性,超声声像图对于评价乳腺癌的预后有较为重要的意义。Objective To investigate the correlation between appearance of ultrasound and immunochemical index of breast cancer. Methods Ultrasonography was performed on seventy cases of breast cancer before operation, the samples were collected for immunochemistry examination , then correlation between sonograms and immunochemical indexes was analyzed. Results The positive expression rate of ER, PR in lesion with the spicule sign was higher than that without the spicule sign, there was no significant difference. The positive expression rate of C - erbB - 2, P53 and Ki - 67 was lower than that without the spicule sign, and there was significant difference between P53 and Ki- 67; There was no significant difference between positive expression rate of ER, PR, P53,C - erbB - 2 and Ki - 67 and homogeneity of tumor internal echo. The positive expression rate of ER, PR in tumors with calcification was lower than that without the microcalcification, and there was significant difference. The positive expression rate of C - erbB - 2, Ki - 67 and P53 was higher than that without microcalcification, and there was no significant difference. Conclusion There is correlation between appearance of ultrasound and immunochemical index of breast cancer, and sonograms has important value for evaluating prognosis of breast tumor.
