摘　　要：目的探讨行胰十二指肠切除术的老年患者围手术期的临床诊治特点。方法回顾分析华东医院普外科胰腺外科组2002年8月至今施行的246例胰十二指肠切除术临床资料,比较老年组和非老年组患者术后并发症的异同。结果老年组患者肺部感染、心衰和谵妄的发生率高于非老年组,两组患者吻合口漏和死亡率的差异无统计学意义,结论高龄不是胰十二指肠切除术的反指征,对老年患者当加强围手术期监护。Objectives To study the clinical characteristics of aged patients who underwent pancreaticoduodenectomy with respect to diagnosis and treatment. Methods Clinical data of 246 patients with pancreatic cancer who received pancreaticoduodenectomy in Huadong Hospital since August 2002 were retrospectively reviewed and analyzed to compare differences in postoperative complications between elderly patients and younger patients. Results The morbidity of pulmonary infection, heart failure and delirium in the elderly patient group was significantly higher than that in the younger patient group, while there was no significant difference in anastomotic leakage and mortality between the two groups. Conclusion Age is not a contraindication for pancreaticoduodenectomy, and perioperative care should be intensified in elderly patients.
