氟西汀并心理治疗对围绝经期抑郁症疗效研究

摘　　要：目的比较综合应用氟西汀、性激素及心理治疗与单用性激素治疗围绝经期妇女抑郁症的临床效果。方法选择伴发焦虑/抑郁症状的围绝经期综合征妇女66例,随机分成3组:第1组23人单用性激素替代治疗,第2组23人应用性激素和氟西汀,第3组综合应用氟西汀、性激素及心理治疗。应用调查表、精神障碍诊断量表、Hamilton抑郁量表、Hamilton焦虑量表、副反应量表进行评估。结果三组患者在治疗6周后HAMD评分较治疗前相比差异有统计学意义(P<0.05)。第1组与第2组间、第2组与第3组间无明显差异,但第1组与第3组间疗效差异存在统计学意义(P<0.01)。结论性激素替代治疗虽然对围绝经期伴发焦虑/抑郁症状有效,但联合应用性激素替代疗法、氟西汀及心理治疗更有益于围绝经期综合征患者的恢复。Objective To compare the effect of combination of Fluoxetine, psychotherapy and hormone replacement treatment （HRT） with the effect of HRT only in treating perimenopausal depression. Methods 66 perimenopausal patients with depression were randomly divided into three groups. The first group included 23 patients treated with HRT only,the second group included 23 patients treated with HRT and Fluoxetine, and the third group included 20 patients treated with HRT, Fluoxetine and psychotherapy. The cure effect were evaluated by questionnaire, Diagnostic Schedule of Mental Disorder（ DSMD）, hamilton depression scale （HAMD） ,hamilton anxiety scale（HAMA） and side reaction measuring scale. Results All of the three groups showed significant improvement after 6 weeks treatment in terms of HAMD scores. There was no significant difference between the first group and the second group, the second group and the third group, meanwhile there was significant difference between the first group and the third group （ P 〈 0.05 ）. Conclusions All of the three projects had effects on the perimenopausal patients with anxiety/depression, but combination of HRT, Fluoxetine and psychotherapy showed better effect.
