Carisolv法和车针法治疗乳牙龋病的临床比较

摘　　要：目的比较Carisolv微创法与传统车针法在乳牙龋治疗中的充填效果,对Carisolv法做出临床评价。方法选择66例双侧同名乳磨牙中、深龋患者,共135颗患牙。同一患者一侧为传统车针法去龋充填,一侧为Carisolv法去龋充填。比较两种方法去腐时间、患者接受程度、充填体脱落及继发龋发生情况。结果Carisolv法中、深龋去腐所需时间分别为(12.3±4.2)和(21.3±5.2)min显著长于传统车针法的(6.3±2.1)和(10.5±3.1)min。80.3%的患者认为Carisolv法更为舒适,8.9%的患者认为有异味或时间长难以耐受,10.8%的患者认为两种方法都不好。Carisolv法中龋治疗时,42.8%的患者感到轻微酸痛,并未使用麻药;深龋治疗时,86.2%的患者感到轻微或明显酸痛,10.3%的患者使用麻药。充填后所有患者未见牙龈化学性损害;深龋组充填2周后回访,未出现牙髓明显损害。2年追踪观察,中龋治疗时Carisolv法和车针法在充填体脱落率、继发龋发生率方面无显著性差异(P>0.05);深龋治疗时,Carisolv法充填体脱落率和继发龋发生率均显著低于车针法(P<0.05)。结论乳牙龋病治疗Carisolv法去龋较传统车针法需要更多的时间,但操作温和更易被患者接受,对正常牙体牙髓组织无损害,也未见牙龈化学性损害,深龋治疗时充填体的脱落率和继发龋发生率较车针法显著减少。Objective To compare the result of Carisolv and conventional restoration in cavities of deciduous molar caries. Methods Sixty-six children ,who suffered from deciduous molar caries in both side of maxillary or mandibular, were selected. One side was restoration of conventional, another was carisolv. All the cavity were filled by GC Fuji IX GP. All the children had been observed for 2 years. Result Carisolv method took more time than conventional method, 80.3% children more like Carisolv method. After Carisolv method treatment, no gingival or pulp injury had been found. In the failure rate of middle caries, Carisolv method was 21.93%, conventional 17.2%, there were no significant difference between both sides. The failure rate of deep caries, Carisolv method was 20.6%, conventional 43.3%, there were significant difference. Conclusion Carisolv method took more time in cavity preparation than conventional method, but children more like Carisolv method. Carisolv method for treating deciduous caries was safety. The effective rates of restoration have significant difference between Carisolv method and conventional method.
