游离腓骨肌皮瓣Ⅰ期修复上下颌骨及软组织缺损22例

摘　　要：目的本文结合我科应用游离腓骨肌皮瓣Ⅰ期修复上、下颌骨及软组织缺损的经验,探讨游离腓骨肌皮瓣的制备要点及用途。方法对22例口腔癌累及上、下颌骨的患者,行上、下颌骨及周围软组织切除后,采用游离腓骨肌皮瓣进行上、下颌骨重建及软组织缺损的修复。结果22例中21例游离腓骨肌皮瓣完全成活,1例骨瓣成活、皮瓣坏死。结论对于肿瘤切除后的上、下颌骨及软组织缺损的患者应用腓骨肌皮瓣立即修复是一个可行的、效果良好的方法,能使患者的生存质量得以提高。Objective To summarize the experiences of using fibula free flap for the reconstruction of maxilla and mandibular defects. Methods Twenty-two patients with maxilla or mandibular defects were reconstructed with fibula free flap. The fibula free flaps were cut into segments and bent to appropriate shapes according to the features of the maxilla and mandibular defects. Then they were be fixed precisely by the titanium miniplates. Results Twenty-one of the 22 patients with fibula free flaps survived well without surgery failure. The wounds healed well. Patients were satisfied with their facial symmetry. Conclusion The fibula free flap is an ideal material for the reconstruction of maxilla and mandibular defects; it improves life quality of the patients well.
