急性缺血性卒中后血清白蛋白甘油三酯水平与预后的研究

摘　　要：目的:探讨急性缺血性卒中患者入院时血清白蛋白(Alb)、甘油三酯(TG)水平对预后的影响,了解影响急性缺血性卒中患者Alb、TG水平的因素。方法:选择急性缺血性卒中患者216例,均在入院后次晨检测Alb、TG水平。所有患者均在入院时采用美国国立卫生院卒中评分(national institute of health strokescore,NIHSS)进行临床神经功能缺损程度的评定,并在发病1、3、5个月时进行Barthel生活指数(Barthel In-dex,BI)评分,BI作为评价预后的主要指标。结果:低Alb组发病1个月和3个月时BI值低于正常Alb组;高TG患者组发病1个月和3个月BI值明显高于正常/降低TG患者组。应用Logistic多元回归分析发现,影响Alb、TG水平的因素有意识障碍和入院神经功能缺损。结论:意识障碍和入院时病情严重程度影响患者入院时Alb、TG水平;入院时Alb、TG水平可能与缺血性卒中患者发病1个月、3个月预后有关。Objective：To study the serum albumin and triglyceride levels effect on the prognosis and understand the influencing factors of serum albumin and triglyceride levels in the acute ischemic stroke patients. Methods： Choosing 216 patients with acute ischemic stroke. In the hospital after testing Cichen Alb/TG levels of the factors. All patients were tested for clinical neurological deficits adopting the National Hospital Stroke score （national institute of health stroke score） of the United States after admission. Evaluated according to Barthel life index （Barthel Index BI） on the onset of on the first month, the third month, and the fifth month and give the judgement for daily life activities adopting incidence on the first month, the third month, and the fifth month to regard BI as the main targets of the prognosis evaluation. Results： Serum albumin in patients with a low incidence group on the first month, the third month the BI was significantly lower than patients with normalserum albumin ;and patients with high triglyceride group incidence on the first month BI and March was significantly higher than normal/lower triglyceride patient group. Logistic multiple regression analysis found that effect with serum Alb and TG levels has awareness and obstacles to the admission neurological deficit. Conclusion： Sense of obstacles and admission is the severity of patients on the factors of serum Alb, TG levels, Alb, TG levels have relation with the prognosis of onset on the first month and the third month in the acute ischemic stroke patients.
